**DanzStar, LLC – Automatic Payment Policy Summary**

DanzStar, LLC offers an Automatic Payment System for monthly tuition, recital, and costume fees. Payments are charged to the credit or debit card on file on the 15th of each month. A 3.05% card processing convenience fee applies to all transactions.

Tuition Rates for 2025–2026:

1 class/week:** $105/month
2 classes/week:** $210/month
3rd class:** $85/month
Unlimited classes:** $315/month
*A $20 fee will be applied to any declined payment. Students with unpaid tuition will not be permitted to participate in class.*

Recital & Costume Fees:

**Recital Fee:** $395 per dancer; $140 for each additional sibling
**Costume Fee:** $100 per class ($95 for toddler class)
Registrations after **December 1st** will require a meeting with the Director to arrange payments, with all balances due by **February 15, 2026**.

Contemporary/Hip Hop Package $60
Toddler Package: $100

Payment Policy:

By enrolling, you authorize DanzStar to automatically process all payments as listed. DanzStar does not accept tuition or recital payments via CashApp, Zelle, or PayPal.

I understand that DanzStar communicates through email, text messages, and the Band App. I agree to create a Band App account, refrain from posting negative comments or feedback, and to treat all staff members with respect at all times.

I give my consent for my child to participate in a faith-based dance program at DanzStar, where they will engage in Bible studies and learn Bible verses as part of their overall development. I understand that monthly Bible study sessions are held at the DanzStar studio, and I support my child’s involvement in these gatherings.

I acknowledge that DanzStar is rooted in Christian values and that biblical principles will be shared with my child in a loving, respectful, and God-centered manner. The foundation of DanzStar’s faith-based approach is built upon the Three P’s of Faith:

3.)Persistence

These values are designed to inspire and support each student in reaching their fullest potential—both in character and in dance technique.

Parent Acknowledgment

I confirm that I have received and reviewed the DanzStar Parent Handbook (please check for your copy under files in your portal), including the Parent Conduct Guidelines. I understand and agree to adhere to the policies outlined within.

At DanzStar, the health and safety of our dancers is our top priority. To maintain a secure and nurturing environment, we limit class sizes and ensure that every class has a dedicated Teacher’s Assistant.

We have implemented safety protocols based on guidance from law enforcement, which include:

- 24/7 video surveillance
- Emergency procedures
- Locking the studio doors 10 minutes after class begins

In addition, we follow strict health and sanitization procedures throughout our facility. This includes regular disinfection of high-touch areas and providing disinfectant wipes and hand sanitizer for all dancers and families.

We kindly ask that dancers who are feeling unwell remain at home until they are symptom-free, to help prevent the spread of illness.

We also encourage healthy habits—such as proper hygiene, regular exercise, and balanced nutrition—as essential practices that support both wellness and strong performance in dance.

By signing below, I acknowledge and agree to the following terms:

I understand that participation in dance-related activities—whether held in person or online—carries inherent risks of injury. I voluntarily assume full responsibility for any and all risks associated with these activities, whether foreseen or unforeseen.

I hereby release and discharge DanzStar, LLC, along with its directors, officers, employees, volunteers, agents, and representatives, from any and all liabilities, claims, demands, or causes of action arising out of or related to any loss, damage, injury, or illness that may be sustained while participating in any DanzStar-related activities. This includes, but is not limited to, claims resulting from the negligence of DanzStar, LLC or any third party involved.

I further acknowledge that DanzStar, LLC is not responsible for errors, omissions, or failure to act by any third parties.

Should I, my child(ren), or any member of my household cause damage to DanzStar property, equipment, or facilities due to negligence or misconduct, I agree to be financially responsible for any resulting repairs or replacement costs.

I also agree to indemnify and hold harmless DanzStar, LLC from any claims, suits, or legal actions—whether initiated by me, my family, or any third party on my behalf—including associated attorney fees and legal expenses.

I confirm that I have entered into this agreement freely and voluntarily, without any coercion or undue influence. I fully understand its contents and affirm that its terms are clear and unambiguous.

If any provision of this waiver is found to be invalid or unenforceable, the remaining provisions shall continue in full force and effect. Any provision deemed unenforceable shall be modified to the extent necessary to become valid and enforceable.

By signing this form electronically, I affirm that:

- I am at least 18 years of age;
- I have read, understood, and voluntarily agree to this Liability Release Waiver;
- I consent to the use of my electronic initials and/or signature in place of a physical signature.

**Behavior at the Studio and Studio Events**

The behavior of dancers and their families at competitions, conventions, and studio events is a direct reflection of DanzStar’s values and reputation. It is essential that all individuals demonstrate kindness, respect, and professionalism toward everyone in attendance—including participants from other studios.

All communication—verbal or written—about DanzStar, its students, other studios, or event activities should be handled with courtesy and discretion. Inappropriate or disrespectful comments are not acceptable.

The following are strictly prohibited at all DanzStar events and on studio premises:

- Profane or offensive language
- Use or possession of alcohol or illegal substances
- Any behavior that creates an unsafe or hostile environment

Violations of this policy may result in immediate removal from the event and/or dismissal from the program.

We also ask all participants to remain mindful of the presence of children at all times and to help us maintain a family-friendly atmosphere.

Regarding smoking:
For the health and comfort of our dancers and staff, we kindly ask that individuals who smoke do not enter the studio immediately afterward, as the lingering smell can be distracting and triggering for others. Please note that our Director and several team members have asthma, and we must take precautions to maintain a clean, breathable environment for everyone.

Please keep the following in mind to help create a focused, respectful, and fun learning environment:

1.) Be On Time: Arrive 5–10 minutes early. Latecomers may be asked to observe and take notes.
2.) Report Absences: Use the attendance portal to mark any absences in advance.
3.) Come Prepared: Be dressed and ready to begin class on time with proper attire and materials.
4.) Engage Fully: Listen carefully, participate actively, and stay focused throughout class.
5.) Receive Corrections Positively: Feedback helps you grow—receive it with gratitude and apply it.
6.) No Cell Phones: Phones should be silenced and put away during class.
7.) Water Only: Only closed water bottles are allowed in the studio.
8.) Respect the Space: Help keep the studio clean—pick up after yourself and handle equipment with care.
9.) Stay Positive: Bring your best attitude. Be open, kind, and ready to learn.
10.) Give Your Best: Work hard, challenge yourself, and enjoy the process.
11.) Uplift Others: Show respect to classmates, teachers, and staff. Bullying, foul language, and drug use will not be tolerated.
