Studio Policies, Liability Waiver & Registration Agreement
By enrolling a student at Dynamix Performing Arts Academy LLC (“the Studio”), the undersigned parent or legal guardian (“Parent/Guardian”) acknowledges and agrees to the following:

1. ASSUMPTION OF RISK
Parent/Guardian understands that participation in dance, tumbling, acrobatics, and performing arts activities involves inherent risks, including but not limited to physical exertion, falls, repetitive motion, collisions, and exposure to illness.
Parent/Guardian knowingly, voluntarily, and expressly assumes full responsibility for all risks of injury, illness, property loss, or damage arising out of or related to the student’s participation in Studio activities, whether occurring on or off Studio premises.

2. RELEASE, WAIVER & HOLD HARMLESS
Parent/Guardian hereby releases, waives, discharges, and agrees to hold harmless and indemnify Dynamix Performing Arts Academy LLC, its owners, directors, instructors, employees, volunteers, agents, and affiliates from any and all claims, demands, causes of action, damages, or liability of any kind arising out of or related to the student’s participation in Studio programs, except to the extent caused by gross negligence or willful misconduct.

3. MEDICAL AUTHORIZATION
In the event of an accident or medical emergency, Parent/Guardian authorizes the Studio to secure emergency medical treatment for the student if Parent/Guardian cannot be reached. Parent/Guardian accepts full financial responsibility for any medical care provided.
Parent/Guardian agrees to disclose all known medical conditions, allergies, or physical limitations at the time of registration.

4. COMMUNICABLE ILLNESS / PANDEMIC CLAUSE
Parent/Guardian acknowledges that participation in group activities carries an inherent risk of exposure to communicable illnesses, including but not limited to COVID‑19 or similar viruses.
Parent/Guardian voluntarily assumes all risks related to illness exposure and releases the Studio from any claims related to transmission or illness.
The Studio reserves the right to modify schedules, class formats (including virtual instruction), capacity limits, or safety protocols in response to public health concerns or government guidance. Monthly tuition remains due regardless of such modifications, and refunds are not guaranteed.

5. TUITION & PAYMENT POLICY
• Tuition is billed monthly and is due regardless of attendance.
• Missed classes due to illness, vacation, weather, or other absences are non‑refundable.
• Makeup classes are not guaranteed and are subject to availability.
• Late or declined payments may result in late fees or suspension from classes.

6. WITHDRAWAL & REFUND POLICY
• Written notice is required prior to the next billing cycle to discontinue enrollment.
• Tuition, registration fees, and other charges are non‑refundable once processed.
• No refunds will be issued for early withdrawal, missed classes, or Studio closures beyond the Studio’s control.

7. CLASS PLACEMENT & SCHEDULING
Class placement is determined by age, experience, skill level, and instructor discretion. The Studio reserves the right to adjust schedules, substitute instructors, combine classes, or cancel classes as necessary.

8. STUDENT CONDUCT
Students are expected to follow instructions and maintain respectful behavior. The Studio reserves the right to remove a student from class or terminate enrollment for unsafe or disruptive behavior, without refund.

9. DROP‑OFF & PICK‑UP
Parent/Guardian is responsible for timely drop‑off and pick‑up. The Studio is not responsible for students outside of their scheduled class times. Repeated late pick‑ups may result in additional fees.

10. MEDIA RELEASE
Parent/Guardian grants permission for photographs and video of the student’s participation to be used for Studio marketing, promotional materials, and social media.

11. ACKNOWLEDGMENT & ELECTRONIC ACCEPTANCE
Parent/Guardian certifies that they have read, understand, and voluntarily agree to all policies and waivers stated above and that they are the legal guardian of the enrolled student.
