By enrolling in classes or programs, I confirm to review and agree to follow the Latin Ballet of Virginia policies, including but not limited to: - tuition and payment policies - attendance and participation expectations - dress code requirements - student and parent conduct policies - safety and facility guidelines I understand that failure to follow policies may result in removal from class or program participation.
Participation in dance classes, rehearsals, performances, camps, and related activities involves physical activity and inherent risks, including but not limited to slips, falls, muscle strain, fatigue, and injury. By enrolling, I acknowledge that I understand the nature of dance training and voluntarily allow myself or my child to participate. I agree that the participant will follow all instructor guidance and safety instructions. I understand that Latin Ballet of Virginia will take reasonable precautions to provide a safe environment; however, not all risks can be eliminated. Nothing in this agreement is intended to waive rights under applicable Virginia law.
I grant permission for the Latin Ballet of Virginia to photograph, video record, or otherwise document myself or my child during: - classes - rehearsals - performances - workshops - events These materials may be used for: - educational purposes - promotional materials - social media - websites - grant reporting - marketing publications. No compensation will be provided for the use of these materials. If a parent/guardian wishes to opt out of media use, they must submit a written request to the organization.
