LIABILITY WAIVER & RELEASE OF CLAIMS
Aspire Dance Company

1. Assumption of Risk
I understand that participation in dance classes, rehearsals, workshops, performances, competitions, travel, and related activities (“Activities”) at Aspire Dance Company involves inherent risks. These include, but are not limited to:

Slips, trips, falls
Muscle strains, sprains, and injuries
Collisions with other participants, Physical exertion
Use of equipment or props
Travel to and from Activities

I voluntarily assume all risks associated with participation for myself or my child.

2. Release of Liability

In consideration of participation, I hereby release and hold harmless Aspire Dance Company, its owners, instructors, employees, volunteers, independent contractors, representatives, and agents from any and all claims, demands, actions, costs, or liabilities related to:

Personal injury, Illness, Property damage
Accidents or incidents occurring before, during, or after Studio activities

This release applies to all ordinary negligence but does not apply to gross negligence or intentional misconduct.

3. Medical Treatment Authorization

In the event of an injury or medical emergency, I authorize the Studio to obtain medical treatment for me or my child. I assume full financial responsibility for any medical care provided.

4. Health & Safety Acknowledgment

I confirm that the participant:

Is physically able to take part in dance activities
Has no medical conditions that would prevent safe participation
(or that such conditions have been disclosed to the Studio)

I agree to notify the Studio of any changes in health status.

5. Personal Property

I understand that the Studio is not responsible for loss, theft, or damage to personal belongings brought onto the premises or to events.

6. Code of Conduct

I agree that I or my child will follow all rules, instructions, and safety guidelines provided by Aspire Dance Company staff. Failure to comply may result in removal from activities without refund.

7. Binding Agreement

This waiver:

Applies to the participant’s entire involvement with Aspire Dance Company
Remains in effect unless revoked in writing
Is binding on the participant and their parents/guardians, family members, heirs, and legal representatives

8. Acknowledgment & Signature

By signing below, I acknowledge that I have read, understood, and agree to the terms of this Liability Waiver & Release of Claims.

MEDIA RELEASE WAIVER

Aspire Dance Company

1. Authorization for Use of Media

I hereby grant Aspire Dance Company, its owners, directors, instructors, employees, contractors, and representatives (collectively “the Studio”) the irrevocable right and permission to photograph, record, and/or film me or my child during classes, rehearsals, performances, competitions, events, and any other Studio-related activities.

2. Use of Media

I authorize the Studio to use, reproduce, publish, and distribute such images, audio, and video in any media format, including but not limited to:

Social media platforms
The Studio’s website
Printed materials (flyers, posters, brochures)
Advertisements and marketing materials
Educational or promotional videos
Press releases and media coverage

I understand that these materials may be used for promotional, marketing, educational, or informational purposes and that I grant this permission without expectation of compensation.

3. Rights and Ownership

I acknowledge that all photographs, recordings, and footage become the property of Aspire Dance Company. I waive any right to inspect or approve the finished product or any use in which my or my child’s likeness appears.

4. No Liability

I release and hold harmless the Studio from any claims, damages, or liability arising from the use of media containing my or my child’s likeness, including claims of invasion of privacy, defamation, or misrepresentation.

5. Duration

This authorization is continuous and remains in effect unless revoked in writing. Revocation applies only to future content and does not require removal of previously published materials.

6. Opt-Out Option

□ I DO NOT grant permission for my or my child’s image to be used in any Studio-related media.
(If this box is checked, the Studio will make reasonable efforts to exclude the participant from identifiable media.)
