925 Dance Collective
Agreement to Participate and Liability Release

By enrolling myself or my child (“Participant”) in any classes, rehearsals, workshops, or events with 925 Dance Collective (“925 Dance Collective,” “the Studio”), I acknowledge and agree to the following terms and conditions:

1. Assumption of Risk

I understand that participation in dance classes, conditioning, acrobatics, rehearsals, and performance activities involves inherent risks of injury, including but not limited to slips, falls, sprains, strains, fractures, and other physical injuries.

Parents and guardians acknowledge that 925 Dance Collective is not responsible for supervision of dancers outside of scheduled class times, including before class begins and after class ends.

By signing below, I voluntarily assume all risks associated with participation for myself and/or my child.

2. Release of Liability

I, on behalf of myself, my child, and our heirs and assigns, release and hold harmless 925 Dance Collective, its directors, officers, employees, instructors, volunteers, and agents from any and all claims, demands, actions, or causes of action arising out of participation in studio programs or the use of studio facilities, whether arising from negligence or otherwise.

This release includes, but is not limited to, injury, illness (including communicable diseases), or property damage sustained before, during, or after participation.

3. Medical Treatment Consent

In the event of a medical emergency, I authorize 925 Dance Collective staff to obtain emergency medical care for my child or myself as deemed necessary.

I agree to be fully responsible for any resulting medical expenses incurred on behalf of the participant.

4. Financial Responsibility & AutoPay Authorization

I understand and agree to the tuition, AutoPay, and payment policies as outlined in the 925 Dance Collective Parent & Dancer Agreement.

I authorize the studio’s secure payment system through the Dancer & Parent Portal to process automatic payments for tuition, registration, and related fees using the payment method on file.

5. Attendance & Behavior

I agree to support my child’s regular attendance and respectful behavior in class.

925 Dance Collective reserves the right to dismiss a student from participation due to repeated disruptive conduct, unsafe behavior, or violation of studio policies.

6. Public Health Disclosure

I understand that participation in in-person activities carries potential exposure risk to communicable diseases.

I agree to follow all health and safety protocols established by the Studio and acknowledge that 925 Dance Collective cannot guarantee a disease-free environment.

7. Acknowledgment of Understanding

I certify that I have read and fully understand this waiver. I understand that by signing electronically through the Dancer & Parent Portal, I am agreeing to these terms voluntarily and of my own free will.

Privacy & Communication Policy

1. Purpose of this Agreement

925 Dance Collective (“we,” “us,” or “our”) is committed to protecting the privacy and personal information of our students and families.

By signing this agreement, you acknowledge and consent to the collection, use, and limited sharing of personal and contact information necessary for safe and effective participation in our programs.

2. Information We Collect

925 Dance Collective collects and maintains information through our secure studio management system within the Dancer & Parent Portal, including:

• Student and parent/guardian names
• Contact information (email, phone number, mailing address)
• Emergency contact and medical information
• Attendance, class enrollment, and payment records

This information is collected solely to support instruction, communication, safety, and studio operations.

3. How We Use Your Information

Your information is used only for legitimate operational and educational purposes, including:

• Class registration and enrollment management
• Tuition billing and payment processing through the Dancer & Parent Portal
• Emergency and safety communication
• Scheduling and attendance records
• Sending updates about studio programs, performances, or closures

925 Dance Collective does not sell personal information or share personal data with third parties for marketing purposes.

Certain service providers (such as payment processors or studio management platforms) may process information as necessary to support studio operations.

4. Data Storage and Security

All data is stored securely within the Dancer & Parent Portal system and protected through administrative, technical, and physical safeguards.

Access to participant records is limited to authorized staff who require this information to perform their duties.

925 Dance Collective retains personal information only as long as necessary to support active enrollment or as required by law.

5. Communications Consent

By signing this agreement, I consent to receive essential communications from 925 Dance Collective through:

• Email
• Text message
• Phone
• Notifications through the Dancer & Parent Portal or the 925 Dance Collective App

These communications may include:

• class schedules
• tuition notices
• performance information
• studio updates or closures
• policy reminders

Families may unsubscribe from non-essential promotional messages at any time, but may still receive communications related to safety, billing, or class participation.

6. Privacy Rights

As a California resident, I understand that I have the right to:

• request access to personal data maintained by 925 Dance Collective
• request correction of inaccurate information
• request deletion of personal data where legally permitted
• withdraw consent to communications at any time

7. Policy Updates

This Privacy & Communication Policy may be updated periodically. The most current version will always be available through the Dancer & Parent Portal.

8. Acknowledgment of Understanding

By electronically signing this agreement through the Dancer & Parent Portal, I acknowledge that I have read and understood the 925 Dance Collective Privacy & Communication Policy and consent to the collection and use of personal information as described above.
