The Foundation Dance Complex
Waiver and Release of Liability
By agreeing to this waiver during registration, I acknowledge and agree to the following terms:
1. Assumption of Risk and Waiver of Liability
I understand that participation in dance, fitness, tumbling/acrobatics, and related physical activities involves inherent risks, including but not limited to the risk of injury, illness, or property damage. I voluntarily assume all such risks for myself or my child and release The Foundation Dance Complex, including its owners, staff, contractors, and affiliates, from any and all liability, claims, or demands arising from participation in any classes, rehearsals, events, or activities, whether held on or off studio premises.
2. Medical Authorization
In the event of a medical emergency, I authorize The Foundation Dance Complex to seek emergency medical treatment for myself or my child and understand that I am financially responsible for any medical services rendered.
3. Media Release
I grant permission to The Foundation Dance Complex to use photographs and/or video footage of myself or my child for marketing, promotional, and educational purposes. I understand that names may or may not be used, and that no compensation will be provided for the use of such media.
4. Studio Policies
I agree to adhere to all policies set forth by The Foundation Dance Complex, including but not limited to dress code, attendance, class conduct, tuition payments, and health/safety protocols. I understand that tuition and fees are non-refundable and non-transferable.
5. Consent and Acknowledgement
By completing this registration, I confirm that I have read, understood, and agreed to the terms outlined in this waiver. If registering a minor, I certify that I am the legal parent or guardian and have the authority to provide this consent.
