Acknowledgment of Risks
I/we (the dancer and parent/guardian) realize that participation in dance classes and activities could involve possible injury. By signing below, I/we assume all risks related to the use of any and all spaces used by THE DANCE LAB LLC.

Release of Liability
I/we agree to release and hold harmless THE DANCE LAB LLC including its teachers, dancers, staff members, and facilities from any cause of action, claims, or demands now and in the future. I/we will not hold THE DANCE LAB LLC liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes. Furthermore, I/we agree to obey THE DANCE LAB LLC’s Policies and Procedures and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by THE DANCE LAB LLC.

Media Release
I/we understand that the student may be photographed or videotaped randomly throughout the year. I/we grant THE DANCE LAB LLC perpetually, exclusively, and for all media the right to use and incorporate photographs or video footage taken as a result of participation in activities at THE DANCE LAB LLC. No compensation will be allotted for the use of these photographs or videotapes.

Medical Release
I/we request and authorize that in my absence the student/participant be admitted to any hospital or medical facility for diagnosis and/or treatment. I/we authorize Doctors of Medicine, or other such licensed technicians or nurses, to perform any necessary procedures. I/we authorize THE DANCE LAB LLC, including its teachers and staff members, to act for the student/participant according to their best judgment in providing or arranging emergency care in any emergency circumstance requiring medical attention. I/we understand and agree that the child’s parents or legal guardians shall be responsible for any expenses incurred.

The Dance Lab LLC is committed to protecting the privacy and personal information of our students and their families. By enrolling, I acknowledge that the studio may collect and store personal data such as names, contact information, medical details, payment methods, and media releases for the purpose of program administration, safety, communication, and billing. This information will be used solely by The Dance Lab LLC and will not be shared, sold, or distributed to outside parties without prior consent, except when required by law or for essential business services (e.g., payment processing). All records are securely maintained, and access is limited to authorized personnel. I understand that by participating in The Dance Lab LLC programs, I consent to this data collection and use, and I accept the studio’s privacy practices as a condition of enrollment.
