LATIN ELEMENTS DANCE COMPANY, LLC (LEDC)
General Liability Waiver and Release
Effective Date: Date Signed or Electronically Acknowledged
This General Liability Waiver and Release form is a legally binding agreement between Latin Elements Dance Company, LLC (LEDC), located in Norwalk, Connecticut, and the undersigned participant (or their parent/guardian if the participant is a minor). By signing or electronically acknowledging this waiver, the participant (or their parent/guardian) accepts the terms outlined herein.
1. Assumption of Risk
I understand and acknowledge that participation in LEDC’s dance classes, rehearsals, competitions, travel-related activities, and other events involves inherent risks, including physical injury, illness, emotional distress, or property damage.
I voluntarily assume all risks associated with these activities, whether occurring on LEDC’s premises or at third-party locations.
2. Release of Liability
I release and hold harmless Latin Elements Dance Company, LLC, its owner Steven Lopez, employees, contractors, agents, affiliates, and third-party vendors from any and all claims, demands, actions, or causes of action arising out of participation in LEDC activities.
This includes claims related to:
Bodily injury or harm.
Communicable diseases (e.g., COVID-19 or other illnesses).
Loss, theft, or damage to personal property.
Acts of third parties or unforeseen circumstances.
3. Non-Transferability
This waiver is specific to the participant identified below and cannot be transferred to any other individual.
4. Medical Emergencies
In the event of a medical emergency, I authorize LEDC staff or representatives to seek medical treatment for the participant.
I agree to assume full financial responsibility for any medical care or transportation costs incurred.
5. No Legal Action
I agree not to bring any legal action against Latin Elements Dance Company, LLC, its owner, staff, or affiliates for claims arising out of participation in any activity, except in cases of proven gross negligence or willful misconduct.
6. Personal Property
LEDC is not responsible for the loss, theft, or damage of any personal property brought to the studio or to any LEDC-affiliated event.
7. Governing Law
This waiver is governed by and construed under the laws of the State of Connecticut. Any disputes shall be resolved exclusively in the courts of Fairfield County, Connecticut.
8. Effective Date
This waiver becomes effective on the date it is signed or electronically acknowledged by the participant (or parent/guardian if the participant is a minor).
9. Agreement Scope
This waiver applies indefinitely to all LEDC activities, including future classes, competitions, and events, unless explicitly revoked in writing.
