Latin Elements Dance Company, LLC (LEDC) Enrollment Agreement

Effective Date: Date of electronic acknowledgment in registration portal

By completing enrollment, I confirm that I am either (a) the parent/legal guardian of the minor student being registered, or (b) an adult student (18 or older) registering myself. In either case, I acknowledge and agree to the following terms:

1. Assumption of Risk & Liability Waiver

I understand that participation in dance classes, rehearsals, competitions, performances, travel, and other LEDC activities involves inherent risks, including but not limited to physical injury, illness (including communicable diseases), emotional distress, or property damage.

I voluntarily assume all risks and release and hold harmless Latin Elements Dance Company, LLC (LEDC), its owner Steven Lopez, instructors, employees, contractors, agents, affiliates, and third-party vendors from any and all liability, claims, or demands related to participation in LEDC activities, except in cases of gross negligence or willful misconduct.

2. Medical Emergencies

For students under 18: I authorize LEDC staff or representatives to seek emergency medical treatment if I cannot be reached and accept full financial responsibility for any resulting care.

For adult students: I acknowledge that I am responsible for my own medical care and costs.

3. Tuition, Payments & Fees

No Refunds: All tuition, registration fees, costume fees, recital fees, drop-in fees, class cards, workshops, and competition fees are non-refundable. This applies to all programs (youth and adult), regardless of class attendance, withdrawal, dismissal, absence, or scheduling conflicts.

Tuition for youth programs is billed according to LEDC’s published youth tuition schedule.

Adult programs (drop-ins, class cards, workshops) are billed according to LEDC’s published adult rates.

Tuition and fees are due as published and must be paid in full to maintain active enrollment.

Late payments (youth program only) are subject to a $40 late fee if not received by the 7th of the month.

LEDC reserves the right to suspend or terminate enrollment for accounts more than 30 days overdue.

4. Withdrawals & Cancellations (Youth Program Only)

Written notice is required to withdraw from youth classes.

Tuition is not prorated for early withdrawal, except when permitted under LEDC’s published prorating policy for new enrollments during the season.

Make-up classes may be offered at LEDC’s discretion and according to published policies.

5. Code of Conduct & Studio Policies (All Students)

All students and families agree to uphold LEDC’s standards:

Treat instructors, staff, and peers with respect.

Follow attendance, punctuality, and dress code requirements.

Parents/guardians are responsible for timely drop-off and pick-up of minors.

Bullying, harassment, or disruptive behavior will not be tolerated.

LEDC reserves the right to dismiss any student/family who repeatedly violates studio policies.

6. Personal Property

LEDC is not responsible for the loss, theft, or damage of personal items brought to the studio or events.

7. Privacy & Data Handling

I understand that LEDC collects basic personal information (name, contact details, emergency contacts, and payment information) for the purpose of processing registrations, communicating about classes/events, and ensuring student safety.

Payment details are processed securely by third-party payment processors.

LEDC does not sell personal information to third parties.

I am responsible for ensuring my contact and emergency information remains current.

8. Agreement Scope & Duration

This agreement applies to all LEDC activities, including current and future classes, events, and performances, unless revoked in writing. It does not need to be renewed each year; however, LEDC reserves the right to require re-acknowledgment at the start of a new season if policies or procedures are updated.

9. Acknowledgment

By electronically acknowledging this Enrollment Agreement, I confirm that I have read, understood, and agree to its terms. This agreement is binding whether I am registering myself as an adult student or my child as their parent/guardian.
