Ursula Smith Dance Registration Agreement

Welcome to Ursula Smith Dance!

By creating an account and registering for classes, I acknowledge that I am entering into a registration agreement with Ursula Smith Dance.

I understand that, as part of the registration process, I will be asked to review and accept additional studio waivers and policies, including but not limited to the Health & Injury Waiver, Photography & Video Release, Privacy & Confidentiality Policy, and other studio policies applicable to my student's enrollment.

I understand that enrollment with Ursula Smith Dance is a financial commitment. I agree to pay all tuition, registration fees, recital fees, costume fees, performance fees, and any other applicable charges by their stated due dates.

I understand that all tuition and fees are non-refundable and non-transferable. Withdrawal from the program does not release me from my financial obligation. Tuition remains the responsibility of the account holder until a written withdrawal request has been submitted and processed by Ursula Smith Dance.

I authorize Ursula Smith Dance to charge the payment method on file for scheduled tuition payments, applicable fees, and any authorized charges associated with my student's enrollment. I understand that a convenience fee may be applied to applicable electronic transactions.

I acknowledge that it is my responsibility as the primary account holder to read all emails, invoices, newsletters, statements, and other communications sent by Ursula Smith Dance. Failure to read or receive studio communications does not waive payment obligations, deadlines, rehearsal requirements, studio policies, or other responsibilities.

I understand that Ursula Smith Dance reserves the right to amend or update its policies, tuition, fees, procedures, and handbook at any time. Continued enrollment and participation constitute acceptance of those changes.

I certify that I am the parent, legal guardian, or adult participant authorized to complete this registration and agree to be bound by the policies of Ursula Smith Dance
