Sherry Martin Dance – General Terms of Service
2025–2026 Dance Season
These Terms of Service are entered into between the enrolling party and Sherry Martin Dance, a DBA of KELLYVONNE LLC.
By enrolling in classes, programs, or events at Sherry Martin Dance, you (the Parent/Guardian of a minor participant, and/or the student if 18 years of age or older) agree to the following Terms of Service. These terms are intended to promote a safe, respectful, and transparent environment for all dancers, families, and staff.
Acknowledgment of Studio Policies
I confirm that I have read and understand all studio important dates, pricing, attendance expectations, and policies as outlined in the official Sherry Martin Dance Information, Pricing & Policies for the 2025–26 Season located on our website at sherrymartindance.com
I understand these policies may be updated during the season and agree to review and adhere to all updates shared by the studio via email and website.
Assumption of Risk & Liability Waiver
I understand that dance and movement classes carry a risk of injury. I hereby voluntarily assume all risks associated with participation in classes, rehearsals, performances, studio events, and related activities. I certify that my child is in good physical health and capable of participating in all enrolled activities.
By signing this agreement, I release Sherry Martin Dance, its owners, employees, contractors, and affiliates from any and all claims of liability for personal injury, illness, or property damage sustained during participation in any studio activities or events, on-site or off-site.
Sherry Martin Dance is not responsible for any lost, stolen, or damaged items on the premises or at performance venues.
Tuition & Payment Terms
I understand that all tuition and fees are paid through the Parent Portal, and that an active credit or debit card must be on file at all times.
If my account becomes more than 30 days past due, my child’s participation in regular classes, performances, and competitions may be suspended until the account is brought current.
I understand that all tuition payments are non-refundable. If my child chooses to withdraw from the program, I must notify the studio by email before the first of the upcoming month, as tuition is automatically processed on the 1st of each month. Failure to notify the studio in advance may result in continued charges.
Photo & Video Release
I grant permission for Sherry Martin Dance to use photographs and/or video of my child for studio marketing purposes, including (but not limited to) the studio website, printed materials, email communications, social media, and recital-related promotions. I understand that no personal information (e.g., last name or age) will be shared without additional consent.
Communication
I agree to keep my contact information (email and phone number) up to date and to regularly check for studio communications. I understand that important updates may be shared through email, text messages, or the studio website.
Instruction at Alternate Facilities
I understand and agree that during the 2025–26 dance season, Sherry Martin Dance may hold classes, rehearsals, or events at alternate locations beyond the primary studio. These may include—but are not limited to—local gyms, community centers, partner studios, or performance venues.
I acknowledge that all Sherry Martin Dance policies, codes of conduct, and liability waivers apply equally at these alternate facilities. I accept that while these spaces are selected to meet safety and instructional standards, Sherry Martin Dance is not liable for any incidents, injuries, or property damage that may occur at these external sites.
I understand that it is my responsibility to transport my child to and from these locations and that supervision is only provided during the scheduled class time.
Birthday Parties & Private Events
I understand that Sherry Martin Dance offers birthday parties and private event services either at its primary studio location or other approved venues. I acknowledge that all general studio policies, including behavior expectations and liability waivers, apply during these events.
Parents/guardians hosting or attending birthday parties are responsible for supervising all non-staffed time and ensuring that all participating children are physically able and willing to engage in the activities. Sherry Martin Dance is not liable for injuries, allergic reactions, or property damage that may occur during the event or as a result of activities or refreshments provided.
A non-refundable deposit is required to reserve the party date, and final payment is due as outlined in the party agreement. Additional time, guests, or special requests may incur added fees.
Behavior Expectations
Sherry Martin Dance is committed to fostering a supportive, respectful, and inclusive environment for all students, families, and staff.
By enrolling, I understand and agree that:
1. All dancers are expected to demonstrate kindness, cooperation, and respect toward their instructors, classmates, and the studio environment.
2. Parents, guardians, and guests are expected to conduct themselves respectfully toward staff, students, and other families at all times.
3. Bullying, aggressive behavior, discriminatory language, or actions that disrupt the learning environment will not be tolerated and may result in disciplinary action or dismissal from the program without refund.
4. Dancers must arrive on time, dressed appropriately, and ready to participate. Repeated disruptive behavior, lack of participation, or failure to follow studio rules may result in removal from class.
The studio reserves the right to dismiss any student or family for failure to comply with these standards, at the discretion of the Sherry Martin Dance Team.
Medical Conditions
Student safety is a top priority at Sherry Martin Dance. Parents/guardians are responsible for providing accurate and up-to-date medical information through the Parent Portal. Any specific concerns or conditions that may impact participation must also be communicated directly to the studio via email.
Please note:
Sherry Martin Dance is not an allergy-free environment. Staff are not permitted to administer any medications, including EpiPens or over-the-counter treatments. Students and their families are solely responsible for managing all personal medical needs, including medications, allergies, and emergency care plans. This policy applies to all classes, rehearsals, performances, and events, both on-site and off-site.
