Sherry Martin Dance – General Terms of Service

2026–2027 Dance Season
These Terms of Service are entered into between the enrolling party and Sherry Martin Dance, a DBA of KELLYVONNE LLC.

By enrolling in classes, programs, or events at Sherry Martin Dance, you (the Parent/Guardian of a minor participant, and/or the student if 18 years of age or older) agree to the following Terms of Service. These terms are intended to promote a safe, respectful, and transparent environment for all dancers, families, and staff.

Acknowledgment of Studio Policies
I confirm that I have read and understand all studio important dates, pricing, attendance expectations, and policies as outlined in the official Sherry Martin Dance Information, Pricing & Policies for the 2026–27 Season located on our Parent Portal.

I understand these policies may be updated during the season and agree to review and adhere to all updates shared by the studio via email, Parent Portal or website.

Assumption of Risk & Liability Waiver
I understand that participation in dance and movement classes and related activities involves inherent risks of injury. I voluntarily assume all risks associated with participation in classes, rehearsals, performances, studio events, and other related activities. I certify that my child and/or I (if I am the participant) are in good physical health and capable of safely participating in all enrolled activities.

By signing this agreement, I voluntarily release, waive, and hold harmless Sherry Martin Dance, KELLYVONNE LLC, its owners, employees, instructors, independent contractors, volunteers, and affiliates from any and all claims, demands, causes of action, damages, losses, or liabilities arising out of or related to my child’s and/or my participation in classes, rehearsals, performances, studio events, or any other studio-sponsored activities, whether on-site or off-site, including claims for personal injury, illness, death, or property damage, except where prohibited by applicable law.

Sherry Martin Dance is not responsible for any lost, stolen, or damaged items on the premises or at performance venues.

Tuition & Payment Terms
I understand that all tuition and fees are paid through the Parent Portal, and that an active credit or debit card must be on file at all times.

If my account becomes more than 30 days past due, my child’s and/or my participation (if I am the participant) in regular classes, performances, competitions, and other studio activities may be suspended until the account is brought current. I understand that overdue accounts are also subject to applicable late fees as outlined in the Sherry Martin Dance Information, Pricing & Policies.

I understand that all tuition payments are non-refundable. If my child chooses to withdraw from the program, I must notify the studio by email before the first of the upcoming month, as tuition is automatically processed on the 1st of each month. Failure to notify the studio in advance may result in continued charges.

Photo & Video Release
I grant permission for Sherry Martin Dance to use photographs and/or video of my child for studio marketing purposes, including (but not limited to) the studio website, printed materials, email communications, social media, and recital-related promotions. I understand that no personal information (e.g., last name or age) will be shared without additional consent.

Communication
I agree to keep my contact information (email and phone number) up to date and to regularly check for studio communications. I understand that important updates may be shared through email, and the Parent Portal.

Birthday Parties & Private Events
I understand that Sherry Martin Dance offers birthday parties and private event services. I acknowledge that all general studio policies, including behavior expectations and liability waivers, apply during these events.

Additional Programs & Events
I understand that certain programs and activities, including but not limited to competition, workshops, performances, special events, travel and parades may require additional rules, policies, participation agreements, or financial commitments. By enrolling in these programs, I agree to comply with any supplemental policies or agreements or communication provided by Sherry Martin Dance.

Behavior Expectations
Sherry Martin Dance is committed to fostering a supportive, respectful, and inclusive environment for all students, families, and staff.

By enrolling, I understand and agree that:

1. All dancers are expected to demonstrate kindness, cooperation, and respect toward their instructors, classmates, and the studio environment.

2. Parents, guardians, and guests are expected to conduct themselves respectfully toward staff, students, and other families at all times.

3. Bullying, aggressive behavior, discriminatory language, or actions that disrupt the learning environment will not be tolerated and may result in disciplinary action or dismissal from the program without refund.

4. Dancers must arrive on time, dressed appropriately, and ready to participate. Repeated disruptive behavior, lack of participation, or failure to follow studio rules may result in removal from class.

The studio reserves the right to dismiss any student or family for failure to comply with these standards, at the discretion of the Sherry Martin Dance Team.

Medical Conditions
Student safety is a top priority at Sherry Martin Dance. Parents/guardians are responsible for providing accurate and up-to-date medical information through the Parent Portal. Any specific concerns or conditions that may impact participation must also be communicated directly to the studio via email.

Medical Emergencies
In the event of a medical emergency, I authorize Sherry Martin Dance staff to obtain emergency medical treatment for my child and understand that every reasonable effort will be made to contact me immediately. I accept responsibility for any medical expenses incurred.

Please note:
Sherry Martin Dance is not an allergy-free environment. Staff are not permitted to administer any medications, including EpiPens or over-the-counter treatments. Students and their families are solely responsible for managing all personal medical needs, including medications, allergies, and emergency care plans. This policy applies to all classes, rehearsals, performances, and events, both on-site and off-site.

Unforeseen Circumstances
Sherry Martin Dance reserves the right to modify schedules, instructors, class formats, or temporarily suspend operations due to circumstances beyond its reasonable control, including but not limited to severe weather, public health emergencies, facility issues, or other unforeseen events. Every reasonable effort will be made to provide alternative instruction when possible.

Drop-Off & Pick-Up
Sherry Martin Dance staff supervise students while they are inside the studio and until they exit the exterior studio door. Once a student exits the exterior door, they are immediately under the care and supervision of their parent, guardian, or authorized pick-up person. Sherry Martin Dance is not responsible for supervising students in the parking lot or other exterior areas.

Electronic Acceptance
I understand that checking the acknowledgment box and submitting my registration constitutes my electronic signature and has the same legal force and effect as a handwritten signature.

By checking the box below, I certify that:

1. I am the Parent/Guardian of the child(ren) listed on this registration form and/or a student age 18 or older enrolling in the program.

2. I have fully read and understand the Sherry Martin Dance Information, Pricing & Policies for the 2026–27 season.

3. I have fully read, understood, and agree to the Terms of Service above as they apply to my child(ren)’s and/or my own participation in the program.
