I, the named individual on this Dance Studio Pro account, the parent/guardian of the enrolled participant agree and understand that dance is a potentially hazardous activity. I recognize that there are risks inherent in dance training including but not limited to serious injury. The parent/guardian acknowledges and agrees that by the child's participation and/or by the parent and/or legal guardian signature below, to indemnify and hold harmless Bellaire Dance Academy/Dance Loud Academy LLC, its employees, instructors, officers, directors, agents and insurance company from any liability and claims resulting from any injury that may rise while participating in activities at the Bellaire Dance Academy/Dance Loud LLC. The parent/guardian of the enrolled participant authorizes any representative of Bellaire Dance Academy/Dance Loud Academy LLC to have the participant treated as we deem necessary in any medical emergency during their participation in activities of Bellaire Dance Academy/Dance Loud Academy. Further, the parent/guardian of the enrolled participant agrees to pay all costs associated with medical care and transportation of the participant. To withdraw from any class the studio must receive a 30 day written notice to stop further transactions. I HAVE CAREFULLY READ THE ABOVE LIABILITY RELEASE AND ELECTRONICALLY SIGN IT WITH FULL KNOWLEDGE OF ITS CONTENTS AND SIGNIFICANCE.

DANCE LOUD ACADEMY

PARTICIPATION WAIVER, RELEASE OF LIABILITY, MEDICAL AUTHORIZATION & ELECTRONIC CONSENT

I am the parent and/or legal guardian of the enrolled participant and hereby give permission for my child to participate in classes, rehearsals, performances, competitions, conventions, camps, clinics, special events, acrobatics, tumbling, conditioning, travel-related activities, and all other programs and activities offered by Dance Loud Academy LLC ("Dance Loud Academy").

I understand and acknowledge that participation in dance and related activities involves inherent and unavoidable risks, including but not limited to falls, collisions, physical contact with other participants, equipment failure, overexertion, sprains, strains, broken bones, concussions, permanent injury, illness, disability, paralysis, and, in rare circumstances, death.

I understand that acrobatics, tumbling, aerial skills, tricks, lifts, and other advanced skills carry an increased risk of injury. I voluntarily assume all risks, both known and unknown, associated with my child's participation in any Dance Loud Academy activity.

On behalf of myself, my child, my heirs, assigns, and representatives, I knowingly and voluntarily release, waive, discharge, covenant not to sue, and agree to indemnify and hold harmless Dance Loud Academy LLC, its owners, members, employees, instructors, staff, assistants, volunteers, independent contractors, agents, representatives, affiliated facilities, landlords, event producers, competition organizers, and insurers from any and all claims, demands, causes of action, liabilities, damages, losses, costs, expenses, attorney's fees, or judgments arising from or related to my child's participation in Dance Loud Academy activities, including transportation to and from events, except to the extent caused by gross negligence or willful misconduct where prohibited by applicable law.

I certify that my child is physically capable of participating in dance activities and that I have disclosed any known medical conditions, injuries, allergies, medications, or limitations that could affect participation. I understand that it is my responsibility to keep the studio informed of any changes to my child's medical condition.

In the event of an accident, injury, illness, or medical emergency, I authorize Dance Loud Academy personnel to secure emergency medical treatment for my child if I cannot be reached immediately. I understand and agree that I am solely responsible for all medical, hospital, ambulance, transportation, and related expenses incurred on behalf of my child.

I understand and acknowledge that Dance Loud Academy cannot guarantee the prevention of injuries, illnesses, communicable diseases, accidents, or other unforeseen events.

I further acknowledge that participation in Dance Loud Academy activities is voluntary and that I may withdraw my child at any time subject to the studio's enrollment, withdrawal, tuition, and financial responsibility policies.

I have carefully read this agreement, fully understand its contents, understand that I am giving up certain legal rights on behalf of myself and my child, and voluntarily agree to all terms and conditions contained herein.
