You must be over the age of 18 to register for an account. Only Parents or Students over 18 listed in the Parent Portal will have permission to receive info, receipt, and make changes to account. Parents or Students over 18 listed in the Parent Portal will be responsible for all payments due. If there are multiple family members that will be paying and needing access to the account, the legal guardian of the child may send a notification by email to ICE STUDIOS. Any other family members that have access to the account must be oriented and sign a waiver and follow all guidelines. ICE STUDIOS WILL NOT CONDUCT BUSINESS WITH FAMILY MEMBERS NOT LISTED ON ACCOUNT. ONLY ONE ACCOUNT PER CHILD/FAMILY IS ALLOWED.
It is the responsibility of the Student 18 and older or parents/guardians to stay informed of studios updates. (CHECK EMAILS, TEXT MESSAGES, VOICEMAILS, AND FLYERS OR INFO SHEETS)

I agree to release ICE STUDIOS from any and all liability for the unintentional exposure or harm due to the Coronavirus (COVID-19) ICE STUDIOS agrees to abide by these standards and affirms the same.

By enrolling myself or my child at ICE Studios School of Dance, I agree to follow all studio policies. Tuition is due on the 1st of each month, and a $10 late fee may be added after the 5th. Students with past-due balances may be unable to participate until the account is current. All registration fees, tuition payments, costume fees, competition fees, and other payments are non-refundable unless otherwise stated in writing by ICE Studios. Students must arrive on time, check in through the DanceStudio-Pro app or with their instructor, follow all class and studio rules, and treat instructors, staff, classmates, and the facility with respect. Bullying, fighting, disruptive behavior, and unsupervised roaming are not allowed. Younger students may not be left unattended, and students waiting for pick-up must remain in the designated waiting area. A late pick-up fee may apply after 10 minutes. No food is allowed in the dance studios; water and approved light snacks are permitted in designated areas. Phones must be on silent and put away during class. ICE Studios is not responsible for lost, stolen, or damaged personal belongings. I understand that participation in dance, tumbling, rehearsals, performances, and other physical activities involves a risk of injury, and I voluntarily allow my child to participate. In an emergency, I authorize ICE Studios to contact emergency services and seek necessary medical care if I cannot be reached. ICE Studios reserves the right to refuse service, suspend participation, or dismiss a student or family for nonpayment, unsafe or disruptive behavior, harassment, repeated policy violations, or conduct that negatively affects the safety and well-being of the studio community, to the extent permitted by law. I understand that ICE Studios communicates important information through email, text, and the parent portal, and I am responsible for keeping my contact information current and reviewing studio communications. Stopping attendance does not automatically withdraw a student; written notice is required. By agreeing below, I acknowledge that I have read, understand, and agree to ICE Studios School of Dance’s waivers and policies.

By enrolling your student, you confirm you are the legal parent/guardian and agree to the following policies.

1. Tuition & Payments

* Tuition is due on the 1st of each month. Summer tuition is a one-time payment.
* A $10 late fee is added after the 5th of the month.
* Students with past-due accounts may not participate in class, rehearsals, performances, or receive costumes until the account is current.
* Tuition is not prorated or refunded for absences, holidays, weather closures, or withdrawals.
* Returned checks are subject to a $25 fee.
* Only the legal guardian listed in the Parent Portal may make account changes unless written authorization is provided.

2. Attendance & Withdrawals

* Students should arrive 5–10 minutes early, dressed and ready for class.
* Absences should be reported through the Parent Portal.
* Students with 3 consecutive no-call/no-show absences may be removed from the class roster.
* Withdrawals must be submitted in writing.

3. Arrival & Pick-Up

* Parents must check students in using the DanceStudio-Pro app before class.
* Students ages 10 and under must be escorted into the building and picked up promptly.
* Students 11+ may enter independently with parent permission.
* A $7 late pick-up fee applies for every 15 minutes after class ends.

4. Studio Rules

* ICE STUDIOS is a nut-free facility.
* Water only inside dance rooms.
* Label all belongings. ICE STUDIOS is not responsible for lost or stolen items.
* Students must remain in designated studio areas and may not roam the building.

5. Dress Code

* Proper dance attire and shoes are required for every class.
* No jeans or inappropriate clothing.
* After three dress code reminders, students may be asked to sit out until properly dressed.

6. Parents & Communication
* Parents and visitors are not permitted in classrooms during instruction except during designated Watch Weeks.
* Please do not interrupt instructors while class is in session.
* Students must keep cell phones put away during class.
* Parents are responsible for reading studio emails, texts, and website updates.

7. Costume Fees (Fall Season)
* Costume fees are non-refundable and due by November 5 or must be on an approved payment plan.
* Students with unpaid costume balances may not participate in recital-related activities or receive costumes.

8. Conduct
* Students, parents, and visitors are expected to treat everyone with respect.
* Disruptive behavior, profanity, harassment, or failure to follow studio policies may result in dismissal or removal from the program.
* Video recording classes is prohibited without written permission.

9. Photo & Media Release
By enrolling, you grant ICE STUDIOS permission to use photos and videos of your student for promotional materials, social media, advertising, and the studio website without compensation.

10. Liability & Medical Authorization

* You certify your student is physically able to participate in dance activities.
* You authorize ICE STUDIOS to obtain emergency medical care if necessary and accept responsibility for related expenses.
* You understand dance involves physical risks and release ICE STUDIOS, its staff, volunteers, and facility partners from liability for injuries, accidents, property loss, or exposure to common communicable illnesses while participating in studio activities.

11. Acknowledgment
By completing registration, you acknowledge that you have read and agree to these policies. ICE STUDIOS is not responsible for missed deadlines, incomplete registrations, full classes, or missed communications due to outdated contact information.

Recital Costume & Performance Agreement

I understand that recital costume fees are separate from monthly tuition and are required for any student participating in the annual recital.

I acknowledge and agree to the following:

* Monthly tuition does not include recital costume fees.
* Costume fees are based on the number of recital classes my student is enrolled in.
* Costume fees will be billed during the fall season and are due by November 5, unless I have an approved payment plan with the studio.
* If my student’s costume fee is not paid by the required deadline, my student may not receive their costume and may not be permitted to participate in rehearsals, recital performances, photos, or other recital-related activities until the balance is paid in full.
* Recital tickets are sold separately and are not included with tuition or costume fees.
* It is my responsibility to purchase recital tickets during the scheduled ticket sale. ICE STUDIOS is not responsible if tickets sell out before I make my purchase.
* I understand that it is my responsibility to meet all recital payment deadlines and to monitor studio emails and announcements regarding recital information.
