I authorize and consent that Center Stage shall have the absolute right to publish, use, sell, or assign any and all photographs taken of me and of my children, and any video as a participant(s) in any center stage events and production(s).

I/we realize that participation in dance classes and activities could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by Center Stage Dance and Performing Arts. I/we agree to release and hold harmless Center Stage Dance and Performing Arts including its teachers, dancers, staff members, and facilities used by both entities from any cause of action, claims, or demands now and in the future. I/we will not hold Center Stage Dance and Performing Arts liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes. Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by Center Stage Dance and Performing Arts. I understand that Center Stage Dance and Performing Arts is a licensed and insured business. In the event that I/we should observe any unsafe conduct or conditions before, during or after my/our classes, I/we agree to report the unsafe conduct or conditions to the Owner, Director, Instructor or Staff member as soon as possible.

I understand the monthly tuition installment is due on the 1st of each month (Not including specialty no charge classes). If I elect the monthly budget plan, the tuition is based on the entire season's tuition and is divided into equal payments which will be paid regardless of the number of classes per month. A late fee of $30 is due after the 7th of each month & will be added to my account and will continue to accru until paid. Any account balance past 60 days is subject to collections if left unaddressed. You must give 30 days notice, when dropping a class to avoid being charged for the following month. There are no refunds or prorated credits for missed classes, no exceptions. Missed classes must be made-up within child's level within 30 days, after which time the missed classes will be non-redeemable. All accounts must also be paid up-to-date to participate in recitals, community performances and/or competitions. There is a $50 service fee for all returned checks. Dropped students may re-enter a class only after fully re-registering, provided there is still space available. Center Stage is not liable for any injury sustained during classes or at the school nor is Center Stage responsible for any lost or stolen items.
