Assumption of Risk, Waiver of Liability & Privacy Policy

1. ASSUMPTION OF RISK
I understand that participation in dance, movement, and performance-based activities at Kingston School of Dance (KSD), located at the Tett Centre for Creativity and Learning, involves inherent risks, including but not limited to physical injury, sprains, strains, fractures, or other bodily harm.
**I (or my child) voluntarily assume all such risks associated with participation in classes, rehearsals, performances, events, and intensives offered by KSD, whether in-studio, off-site, or virtual. I understand that the staff will take all reasonable precautions to ensure a safe environment but that risk cannot be fully eliminated.

2. RELEASE OF LIABILITY
In consideration of being permitted to participate in programming at Kingston School of Dance, I hereby release and hold harmless KSD, its faculty, staff, artistic directors, volunteers, and the Tett Centre for Creativity and Learning from any and all liability for personal injury, illness, loss, or damage to person or property.
**This release applies to all Kingston School of Dance activities on or off premises, including travel to performances, workshops, and community engagements.

3. MEDICAL AUTHORIZATION
In the event of a medical emergency and if a parent/guardian or emergency contact cannot be reached, I authorize the staff of Kingston School of Dance to seek and consent to emergency medical care for myself or my child as needed.
**I accept full financial responsibility for any resulting medical treatment or services.

4. PRIVACY & MEDIA CONSENT POLICY
At Kingston School of Dance, we are committed to protecting the privacy and dignity of all students and families.
Upon registration or participation, we collect:
- Student and parent/guardian names and contact info
- Birthdates and emergency contacts
- Class history, attendance, and voluntary medical notes
- Payment and registration information via Studio Pro
**This information is used solely for administrative, instructional, and safety-related purposes. It is never sold or shared externally.

Photo/Video Consent:
Occasionally, KSD staff or authorized representatives may photograph or film classes, performances, or events at the Tett Centre. These images may be used for:
- Our website or newsletters
- Social media and event promotion
- Grant applications or community outreach
**If you do not wish for your child or yourself to appear in media, you may opt out by emailing the office or indicating your preference during registration. We respect and fully honor these requests.

Third-Party Software:
To streamline our operations and protect data, KSD uses secure and trusted platforms such as:
- Studio Pro (registration and class management)
- Mailchimp (email communication)
- Google Forms (surveys and digital agreements)
**All third-party platforms follow industry standards for privacy and security.

5. CODE OF CONDUCT
KSD fosters a respectful, inclusive, and supportive community. All students and families are expected to:
- Treat instructors, staff, and peers with kindness and respect
- Follow studio rules and staff direction
- Maintain appropriate behavior in all studio-affiliated spaces
**Failure to adhere to this code may result in removal from programming without refund.

6. ACKNOWLEDGEMENT & AGREEMENT
By signing to this below (or by submitting online registration through our secure system), I confirm that:
- I have read and understand this policy in full
- I agree to the terms outlined on behalf of myself or my child
- I understand this waiver remains valid for the current program year or until updated in writing by Kingston School of Dance

At Kingston School of Dance, we strive to make quality dance education accessible and manageable for families. Our structured payment plan for youth programming bills tuition on a monthly basis, helping ease financial planning throughout each term. A non-refundable $25 annual registration fee is required at the start of Term 1 for all students, including those in adult classes.
Adult students must pay in full at the time of registration for sessional or drop-in classes. Refunds are limited and subject to the timelines outlined in our payment policies.
We appreciate your cooperation in adhering to these policies, which allow us to maintain smooth operations and continue providing exceptional programming.

Important Payment Policies
Outstanding Balances: Students with unpaid balances will not be permitted to register for Term 2 until their balance is at $0, including adult students. Additionally, students with outstanding balances may not be eligible to participate in the year-end recital.

Year-End Performance: Participation in the year-end performance is conditional on full payment of all Term 2 (spring) fees. If payments are incomplete, eligibility to perform will be reviewed and may be revoked.

Fee Structure & Refund Policies:
Full Session Adult Classes:
- All adult class fees must be paid in full at the time of registration.
- Refund Policy: If a withdrawal or change is made within the first 3 weeks, the student will be charged for 1 month of classes. After the third week, no refunds will be offered.
- Adult classes will be broken up into 3 seasons Fall, Winter & Spring

Adult Drop-In Classes (Including Dance Card Users):
- Drop-in classes must be paid at the time of registration.
- If an adult books a class and does not attend, they will be charged the full class fee. This applies to both single-class and dance card drop-ins.

Youth/Student Classes:
- A minimum payment of 25% of tuition due at registration to secure your space in the class.
- Remaining balance can be split into up to 3 monthly payments due on the first of each month (September, October, November)
- Students with an unpaid balance after Nov. 1 will not be eligible for Term 2 registration.

Refund Policy:
- If a student withdraws within the first 3 weeks of classes, they will be charged for the full first month.
- After the third week, the student is committed for the remainder of the term and responsible for the full term cost, regardless of attendance.

Additional Policies
Annual Registration Fee: A non-refundable $25 registration fee is charged at the beginning of Term 1 for both adult and youth students.
Performance Fee: Any student participating in the year-end performance in June will incur a performance fee on March 1st of that season, based on the number of classes they are performing in.
After the performance fee is charged no-refunds will be offered if a student decides not to participate.
Costumes: KSD does not charge for costumes. Costumes are provided by the school and the school keeps them to reuse for future years — a tradition we are proud to continue.

- Weekly classes
- Workshops and intensives
- Photo/video shoots
- Live performances
- Studio events and community activities

These images or videos may be used in:
- KSD’s social media and website
- Newsletters and print/digital promotional materials
- Community outreach, grants, or press coverage

We value your privacy and always aim to represent our dancers respectfully and professionally.
By default, you are granting permission for KSD to use images or recordings of yourself or your child(ren) for the purposes listed above.
