During the course of the year, SMPA, may wish to use photographs of students in educational publications or in general media releases on a controlled basis. In accordance with school policy, names of individual students will not be released with any photographs. All other photographs and video taping is strictly prohibited without previous consent. I affirm that the student is covered by health insurance or a medical plan. SMPA will not be held responsible for any claims, cost or liabilities resulting from lack of coverage. New this season: All accounts must have a credit card on file. Auto tuition will be drawn from all accounts on the 25th of the month for the upcoming month. Any tuition received after the 5th of the current month incurs a $15 late fee. If tuition is not paid by the 10th, lessons for your child will be suspended. We accept Visa, MasterCard, and American Express. I agree to be responsible for all tuition payments of the above named student, regardless of attendance, until SMPA receives written notice (via email or letter) 14 days prior to withdrawal. Absolutely NO refunds are granted. I agree to be responsible for all costume fees for the above named dancer, which include deposits and balances paid in full. I must notify school if I choose to opt out of recitals.
