Policies, Waiver & Acknowledgments
By enrolling my student at South Mountain Performing Arts ("SMPA"), I acknowledge and agree to the following:
Media Release & Privacy Policy
I grant permission for SMPA to use photographs and video recordings of my student for educational, promotional, marketing, social media, website, and studio-related purposes. Student names will not be published with photographs without additional consent.
Personal information collected by SMPA, including contact information, emergency contacts, medical information, and payment details, is used solely for studio operations, communication, billing, student safety, and program administration. SMPA does not sell or share personal information with third parties for marketing purposes.
Photography or video recording of classes, rehearsals, or performances by parents or guests is prohibited without prior authorization from SMPA.
Assumption of Risk & Medical Coverage
I understand that participation in dance classes, rehearsals, performances, camps, workshops, and related activities involves physical activity and carries an inherent risk of injury.
I certify that my student is covered by health insurance or a medical plan. I voluntarily assume all risks associated with participation and agree that SMPA, its owners, faculty, staff, and representatives shall not be held liable for any injury, accident, loss, claim, cost, or liability arising from participation in studio activities.
Tuition & Payment Policies
A valid credit card must be maintained on file for all accounts.
Monthly tuition will be automatically processed on the 25th of each month for the upcoming month's tuition.
A $15 late fee will be assessed to accounts with unpaid balances after the 5th of the month. Students with accounts remaining unpaid after the 10th of the month may be suspended from classes until the account is brought current.
Tuition reserves a student's place in class and is not based on attendance. Tuition, registration fees, and other payments are non-refundable.
Withdrawal Policy
I understand that I am responsible for all tuition and fees until SMPA receives written notice of withdrawal via email or letter at least fourteen (14) days prior to the withdrawal date.
Costume & Performance Fees
I agree to be responsible for all costume fees, including deposits and remaining balances, for the above-named student.
Students are automatically included in recital planning unless written notice is provided by the published recital opt-out deadline. Costume fees are non-refundable once costumes have been ordered.
Communication
I consent to receive studio communications via email, text message, phone call, the Studio Pro Portal App, and push notifications regarding schedules, performances, events, billing, studio updates, and emergency notifications.
By completing registration, I acknowledge that I have read, understand, and agree to the policies, waiver, and terms outlined above.
