PARENT RELEASE FORM FOR MEDIA RECORDING

I do hereby grant permission to Dance Works to use the image of my child. Such use includes the display, distribution, publication, transmission, or otherwise use of photographs, images and/or video taken of my child for use in materials that include, but may not be limited to, printed materials such as brochures and newsletters, advertising, videos, and digital images to include websites and social networks.

By agreeing to this form, I understand that I am giving unrestricted permission for my child's image to be used in print, video, and digital media. I understand that these images can be from any/all activities related to Dance Works. I agree that these images may be used by Dance Works for a variety of purposes and that these images may be used without further notifying me. I do understand that the child’s last name will not be used in conjunction with any video or digital images.

*Here at Dance Works we pursue excellence from all of our students appropriate to their age and interest. It is our mission to offer a family oriented recreational dance experience for all ages. We realize that most of the students that come through our studio doors do not wish to be professional dancers but we still strive to make that a possibility. We wish to teach students life skills, as well as, dance skills. Our goal is to have students leave with a well-rounded dance experiences, in which the skills they have been taught can be related to the outside world so they can be used for the rest of their lives. This cannot be accomplished without the support, respect, and assistance from the dancers, their families, and our staff.

*By agreeing to this form, I am indicating I have read the Policies & Procedures packet in its entirety. I am indicating my awareness, understanding, acceptance, and consent of all studio policies, procedures & waivers as detailed in the Dance Works Policies & Procedures Packet. I also understand that these policies may need supplemented or amended from time to time; especially during pandemics and natural disasters. In the event of a mid-season supplementation or amendment, notification will be sent to all active accounts by email.
