I hereby affirm that I have reviewed, understand, and agree to be legally bound by all policies, procedures, terms, and conditions published by Motions Dance Studio on its official website, including but not limited to withdrawal and tuition policies, all of which are incorporated herein by reference.

THIS IS A RELEASE OF LIABILITY, READ BEFORE SIGNING.

This agreement releases MOTIONS DANCE STUDIO LLC, from all liability relating to injuries that may occur on site and/or during one of the events. By signing this agreement, I agree to hold MOTIONS DANCE STUDIO entirely free from any liability, including financial responsibility for injuries, regardless of whether injuries are cause by negligence.

I am the student or the parent or guardian of the student named above (hereinafter “the Student”). I understand that there are known dangers and/or risks associated with the Student’s participation in the above-written program, including all practices, performances and activities associated with it. In consideration of the Student being allowed to participate in the program, I hereby release Motions Dance Studio LLC, Brianna Jensen, and all other independent contractors, other representative, instructors, and/or volunteers from Motions Dance Studio LLC (hereinafter “Motions Dance Studio”) from all claims, damages, actions, lawsuits or judgments that I, the Student, the child’s heirs, executor of representative may have or hereafter have arising out of the Student’s participation in the program. Further, I agree to defend and indemnify Motions Dance Studio from all claims by any person or entity arising out of the Student’s participation in the program.

By signing below, I forfeit all right to bring a suit against MOTIONS DANCE STUDIO LLC for any reason. I will also make every effort to obey safety precautions as listed in writing and as explained to me verbally. I will ask for clarification when needed.
