If the student has any physical or psychological condition of which their instructor should be aware, please notify the school in writing via email: SaugertiesBalletCenter@gmail.com.

In enrolling my child(ren)/self in classes at the Saugerties Ballet Center, I understand that while every effort is made at SBC to provide a safe, accident-free environment, there are inherent risks in participating in any movement activity. I acknowledge that I indemnify and hold harmless Saugerties Ballet Center, its owner and/or instructors from any liability/responsibility for any injury incurred while participating in any activity/classes held by the Saugerties Ballet Center.

I have read and am fully familiar with the contents of this form and agree to the terms thereof.

Photo release

Photo/Video release
I understand that photos and video of my child's class may be used in educational and advertising materials and that there will be no monetary compensation for the use of these photos/videos. Photos/videos may be posted on the Saugerties Ballet Center web site and Facebook /Instagram pages. No names will be used without parent's permission.
