Liability Waiver and Release Form
By enrolling your child in classes at Nan Klinger Excellence in Dance through our online registration system, you agree to the terms outlined in this Liability Waiver and Release Form. Checking the acknowledgment box during registration indicates your acceptance of these terms and serves as your electronic signature.

Acknowledgment of Risk and Waiver of Liability
Assumption of Risk:
I understand and acknowledge that participation in dance classes involves physical activities that may result in injuries, including but not limited to sprains, strains, broken bones, or other physical harm. I accept full responsibility for any risks, known or unknown, associated with participation in classes, rehearsals, performances, or related activities.
Waiver of Liability:
In consideration of my child’s participation in activities at Nan Klinger Excellence in Dance, I, on behalf of myself, my child, and our respective heirs, assigns, and legal representatives, voluntarily release, waive, and discharge Nan Klinger Excellence in Dance, its owners, instructors, employees, and agents from any and all claims, liabilities, demands, or causes of action related to injury, illness, or damages arising out of or connected to participation in studio activities.
Medical Authorization:
In the event of an emergency, I authorize Nan Klinger Excellence in Dance to seek medical attention for my child if I am unavailable and agree to bear any associated costs.
Notification of Medical Conditions:
I agree to inform the studio in writing of any medical conditions, allergies, injuries, or other health concerns that may affect my child’s participation in dance activities.
Ohio Legal Notice:
I understand that under Ohio law, liability waivers are generally enforceable as long as they are entered into knowingly and voluntarily. This waiver does not release Nan Klinger Excellence in Dance from liability for intentional misconduct or gross negligence.
