The privacy of your personal information is very important to Dance Magic Performing Arts Center (DMPAC). We do not sell, rent, or trade your personal information to any third party. We take special care to protect the privacy and security of your information. This policy explains our practices regarding the collection, use, and protection of your personal data.

How We Collect and Use Your Information
--When you register for classes at Dance Magic, we collect personal and student information including:

--Parent/guardian names, addresses, phone numbers, and email addresses

--Emergency contacts

--Student names, birthdates, medical conditions

--Additional comments you may provide

Required fields are marked with a red asterisk on registration forms. This information is necessary for studio operations and is used solely for studio-related purposes. Optional information (not marked with a red asterisk) is collected to help us better serve our families and is also used exclusively for studio business. None of this information will be sold or used for unrelated purposes.

Who Has Access to Your Information
When you register, you’ll create a Parent Portal login using your email address and a password. This login ensures that only you have access to view or update your personal account information.

Dance Magic office staff has access to your personal information strictly for the purpose of managing studio business and communication.

We may access or share your information only under the following circumstances:

--As needed for studio operations (e.g., communication, billing, scheduling)

--If required by law or legal process

--To protect the rights, property, or personal safety of our staff, families, or the public

Your Control Over Your Information

DMPAC Media Release Waiver
As part of our effort to celebrate our dancers and promote studio activities, Dance Magic Performing Arts Center occasionally uses photos and videos featuring students. Please review the media release policy below:

By signing this form, you grant DMPAC permission to use photos or videos of your dancer for:
--Printed materials (brochures, flyers, newsletters, etc.)
--Studio promotions and advertisements (both print and digital)
--Social media platforms and the studio website
--Recap videos and other digital studio content

What it means:
--You give unrestricted permission for your child’s image to appear in print, video, and digital media. Images may be used without further notice.
--Your child’s last name and specific location will never be shared publicly.
--Your support helps us highlight the hard work, joy, and creativity of our dancers while strengthening our studio community and inspiring future performers.

Dance Magic Performing Arts Center Liability Waiver & Release Form

I, the parent or legal guardian (hereafter referred to as the Primary Contact) of the student listed in this registration form, understand that participation in dance classes and related activities may involve a risk of personal injury. While Dance Magic Performing Arts Center (hereafter referred to as DMPAC) takes precautions to ensure a safe environment, accidents and injuries can still occur.

By agreeing to this release, I/we assume all risks associated with participation in any programs, classes, rehearsals, performances, and use of any facilities associated with DMPAC.

I/we agree to release, waive, and hold harmless DMPAC, its instructors, staff, volunteers, dancers, and any facility used by DMPAC from any and all claims, causes of action, or liability for injury, loss, or damage to person or property that may arise from participation in DMPAC activities—whether occurring on-site or off-site.

I/we will not hold DMPAC liable for any personal injury or damage to personal property that may occur before, during, or after classes or events.

Furthermore, I/we agree to abide by all facility and class rules and accept full responsibility for any personal behavior and/or damage caused by myself or my child to any property used by DMPAC.

Lastly, I/we agree to disclose any known physical or medical conditions that may affect participation in dance activities and confirm that my child has received clearance from a medical professional to participate in physical activity, including dance.

By agreeing to this form, I/we acknowledge that I have read, understand, and accept the terms outlined above.

Dance Magic Performing Arts Center Financial Responsibility Agreement
I, the Primary Contact of the dancer named in this registration form, understand that I am solely responsible for any and all tuition, registration, and costume fees associated with the classes in which I have enrolled myself or my child at Dance Magic Performing Arts Center (DMPAC).

By completing this registration, I agree to fulfill all financial obligations before participation in any DMPAC activity. I understand that failure to complete payment may result in suspension from classes, rehearsals, or performances until the account is current.

I acknowledge that any payment arrangements made with other family members—including the other biological parent who may not reside in the same household—are my responsibility to manage. DMPAC cannot invoice or communicate financial matters with anyone other than the Primary Contact, unless:

A separate Payment Agreement Form is completed and submitted by the paying party to dancemagicpac@gmail.com, or

Official court documents are provided, explicitly stating shared financial responsibility between both parents/guardians.

Payment & Refund Policy Acknowledgment
I, the Primary Contact, agree to the following:

1. All tuition and fees are due on the first of each month. Payments may be made via the Parent Portal, cash, or check. Checks should be made payable to DMPAC.

2. If I or my child chooses to withdraw from enrollment after a session has begun, I must notify DMPAC in writing via email at dancemagicpac@gmail.com.

3. DMPAC reserves the right to cancel any class due to insufficient enrollment. In the event of a class cancellation, a full refund will be issued to those who have already paid.

4. If tuition becomes 60 days past due, the student may not participate in classes or performances until the balance is paid in full. Accounts 90 days past due will be turned over to Hamilton County Small Claims Court for collection.

5. I understand that no refunds will be issued for tuition, costume fees, registration fees, or performance fees.

By signing below, I acknowledge that I have read, understood, and agree to the financial responsibility and policies outlined above.
