Storybook Dance Studio LLC is committed to providing a safe, positive, and encouraging environment where students can experience the joy of dance, movement, creativity, and self-expression. By enrolling in classes, workshops, performances, events, or activities provided by Storybook Dance Studio LLC, I acknowledge and agree that dance is a physical activity that involves movement, including but not limited to stretching, jumping, turning, balancing, choreography, floor exercises, and age-appropriate movement activities. I understand that participation in dance may involve certain risks, including but not limited to falls, collisions, muscle strains, sprains, or other injuries, and I voluntarily choose to participate, or allow my child to participate, while understanding and accepting these risks.
I understand that students should participate within their own abilities and that it is my responsibility to notify Storybook Dance Studio LLC of any medical conditions, injuries, physical limitations, allergies, or other concerns that may affect participation. I agree to provide accurate and updated emergency and medical information and understand that it is my responsibility to communicate any changes that may impact my student's safety or ability to participate in classes.
In the event of an emergency, I authorize Storybook Dance Studio LLC staff to take reasonable action to protect the health and safety of the participant, including providing basic first aid when appropriate, contacting emergency medical services, and seeking emergency medical treatment if a parent, guardian, or emergency contact cannot be reached. I understand that Storybook Dance Studio LLC staff are not medical professionals and that I am responsible for any medical expenses resulting from emergency treatment.
To the fullest extent permitted by applicable law, I release, waive, and hold harmless Storybook Dance Studio LLC, its owners, instructors, employees, contractors, volunteers, and representatives from claims, damages, injuries, losses, or expenses arising from participation in Storybook Dance Studio LLC activities, including dance classes, workshops, performances, events, and other studio-related activities. I understand that this release does not apply to circumstances where liability cannot legally be waived.
I understand that maintaining a safe and positive learning environment requires cooperation from students and families. I agree that my child will follow instructor directions, treat instructors, staff, and classmates with respect, and participate in a safe and appropriate manner. I understand that Storybook Dance Studio LLC reserves the right to remove a participant from class if their behavior creates a safety concern for themselves, instructors, staff, or other students.
For Baby & Me and Toddler & Me classes, I understand that a parent, legal guardian, or approved caregiver must remain actively involved throughout the entire class. I understand that these classes are designed as a shared experience between the child, caregiver, and instructor, and that the caregiver is responsible for supervising and supporting their child during participation. I understand that Storybook Dance Studio LLC instructors provide dance instruction and guidance but do not provide childcare or individual supervision.
I understand that Storybook Dance Studio LLC is not responsible for lost, stolen, or damaged personal belongings brought into the studio. I also understand that photography and video permissions are managed separately through the studio's media release process and that any approved use of student images or videos will follow the permissions provided by the parent, guardian, or adult participant.
By electronically signing this agreement, I confirm that I have read and understand this Liability Waiver & Release of Liability, that I understand the risks associated with dance participation, and that I voluntarily agree to allow myself or my child to participate in Storybook Dance Studio LLC activities. I understand that this agreement applies to current and future participation unless updated or replaced by a newer version.
