WAIVER AND PARTICIPATION AGREEMENT
Please read the following in its entirety:
By creating an account and registering for classes, programs, workshops, events, rehearsals, conditioning sessions, wellness activities, or other services through The Dance Collective of Clarksville, I acknowledge and agree to the following:
• I certify that I am at least 18 years of age and legally authorized to register myself and/or any minor participant(s) attached to my account.
• I understand that I am legally and financially responsible for all individuals enrolled under my account, including all tuition, fees, memberships, workshops, purchases, late fees, and other approved charges associated with participation.
• Participation in dance, fitness, conditioning, flexibility, mobility, wellness, and movement based activities involves physical activity and carries an inherent risk of injury.
• I voluntarily choose to participate, or allow my minor child to participate, in activities offered by The Dance Collective of Clarksville.
• I understand that risks may include, but are not limited to, muscle soreness, falls, strains, sprains, aggravation of pre existing conditions, illness, or other injuries that may occur during participation.
• I certify that I or my minor participant are physically capable of participating and have no medical condition that would prevent safe participation unless disclosed to staff.
• I understand it is my responsibility to consult with a physician regarding any medical concerns prior to participation.
• I agree to notify instructors or staff of any injury, pain, discomfort, dizziness, or medical concerns that arise during participation.
• I authorize The Dance Collective of Clarksville staff to seek reasonable emergency medical care if necessary.
• I understand that classes and programs may include physical corrections, spotting, conditioning exercises, flexibility training, floorwork, or movement instruction appropriate to the class or activity.
• I release and hold harmless Total Balance Massage and Wellness, PLLC dba The Dance Collective of Clarksville, its owners, instructors, staff members, independent contractors, volunteers, affiliates, and facility providers from any claims, liabilities, damages, losses, or expenses arising from participation, except in cases of gross negligence or intentional misconduct.
• I understand that enrollment, tuition, memberships, workshops, and participation are subject to studio policies regarding payments, withdrawals, attendance, cancellations, refunds, and conduct.
• I understand that inappropriate, unsafe, disruptive, or disrespectful behavior may result in removal from classes, programs, or events.
• I grant permission for photographs or video taken during classes, events, or performances to be used for promotional or marketing purposes unless written notice is submitted requesting otherwise.
• By completing registration, I acknowledge that I have read, understood, and voluntarily agree to this waiver and participation agreement on behalf of myself and/or my minor participant(s).
