I understand that participation in dance classes, rehearsals, performances, and related activities at Shore Performing Arts involves physical activity and may carry a risk of injury. I voluntarily choose to participate (or that my child participates) and assume all risks associated with participation.

I certify that I (or my child) am physically able to participate in dance activities. I agree to release, waive, and hold harmless the Studio, its owners, instructors, staff, and representatives from any and all liability, claims, demands, injuries, damages, or expenses arising from participation in studio activities, except in cases of gross negligence or intentional misconduct.

I also agree to follow all studio rules, safety instructions, and instructor guidance while participating.
