OORJA DANCEWORKS – PARTICIPATION WAIVER & RELEASE

Welcome to Oorja Danceworks. We’re excited to move with you. Before we begin, we ask that you read and agree to the following:

1. Acknowledgement of Risk

I understand that participation in dance classes, rehearsals, workshops, and related activities at Oorja Danceworks involves physical movement and exertion. This may include, but is not limited to, stretching, cardio activity, floor work, and choreography.

I acknowledge that these activities carry inherent risks, including injury, strain, or physical discomfort. I voluntarily choose to participate and accept all associated risks.

2. Health & Physical Condition

I confirm that I am in suitable physical condition to participate in dance activities.
I agree to:
Listen to my body and work within my limits
Inform the instructor of any injuries, conditions, or concerns
Modify movements as needed

Oorja Danceworks and its instructors are not responsible for any injury resulting from failure to disclose a pre-existing condition.

3. Release of Liability

I hereby release, waive, and discharge Oorja Danceworks, its directors, instructors, artists, and staff from any and all liability, claims, or demands arising from participation in classes or activities, to the fullest extent permitted by law.

4. Emergency Medical Consent

In the event of an emergency, I authorize Oorja Danceworks to seek medical assistance on my behalf. I understand that any associated costs are my responsibility.

5. Community Care & Studio Conduct

Oorja Danceworks is a shared, respectful space.

By participating, I agree to:

Arrive on time and prepared for class
Treat instructors and fellow dancers with respect
Follow studio guidelines related to safety, footwear, and use of space

Oorja reserves the right to refuse participation if conduct compromises the safety or experience of others.

6. Media Release

I grant permission for Oorja Danceworks to capture and use photos or videos taken during classes, workshops, or performances for promotional and archival purposes.

7. Communicable Illness Acknowledgement

I understand that participation in group activities carries a risk of exposure to communicable illnesses. I agree to stay home if unwell and follow any health guidelines in place.

8. Minors

For participants under 18, this waiver must be completed and signed by a parent or legal guardian.

9. Agreement

By signing below, I confirm that I have read, understood, and agree to the terms outlined above.
