Waiver & Policies Agreement
(Applies to all LACI programs, including Studio Colombia Performance Team, Studio 8 by LACI, classes, camps, workshops, and events)
1. Assumption of Risk & Liability Waiver
I understand that participation in dance classes, rehearsals, performances, camps, workshops, travel, and other activities offered by the Latino Arts & Cultural Institute (LACI) involves physical activity and inherent risks, including but not limited to muscle strains, falls, collisions, or other injuries.
I voluntarily assume full responsibility for any risk of injury, loss, or damage arising from participation. I hereby release, waive, and hold harmless the Latino Arts & Cultural Institute (LACI), Studio Colombia, Studio 8 by LACI, its directors, instructors, choreographers, staff, volunteers, partners, contractors, and hosting venues from any and all claims or liability arising out of participation, except in cases of gross negligence or willful misconduct.
2. Medical Treatment Authorization
In the event of an injury or medical emergency, I authorize LACI staff or representatives to seek emergency medical treatment on my behalf (or on behalf of my child, if applicable). I understand that I am solely responsible for any medical expenses incurred.
3. Code of Conduct
All participants are expected to:
Arrive on time and prepared for classes, rehearsals, and performances
Treat instructors, staff, fellow participants, and community partners with respect
Maintain a positive, collaborative, and inclusive environment
Follow choreography, safety guidelines, and program expectations
Respect studio spaces, equipment, and facilities
Disruptive, unsafe, or disrespectful behavior may result in removal from a class, rehearsal, performance, or program without refund.
4. Attendance & Participation Commitment
LACI programs, including Studio Colombia Performance Team, rely on group collaboration and consistency.
Participants are expected to attend scheduled classes, rehearsals, and performances. Excessive absences, late arrivals, or last-minute cancellations may impact participation, casting, performance eligibility, or continued enrollment.
5. Costumes, Materials & Studio Property
Participants may be required to purchase, borrow, or care for costumes, props, or materials.
Borrowed items must be returned in good condition
Participants are responsible for replacing any lost or damaged items
Personal belongings should be clearly labeled
LACI is not responsible for lost, stolen, or damaged personal items.
6. Photo, Video & Media Release
I grant permission for the Latino Arts & Cultural Institute (LACI) to photograph, film, or record participation in classes, rehearsals, performances, and events for promotional, educational, and archival purposes, including but not limited to social media, website, marketing materials, and reports.
I understand that no compensation will be provided for the use of such media.
7. Payments, Refunds & Credits
Program fees, tuition, and registrations are non-refundable unless otherwise stated
Missed classes are not guaranteed to be made up or credited
LACI reserves the right to cancel or reschedule programs as needed
Any exceptions are at the sole discretion of LACI
8. Acknowledgment & Agreement
I acknowledge that I have read, understand, and agree to abide by this Waiver & Policies Agreement. I understand that this agreement applies to all participation in programs offered by the Latino Arts & Cultural Institute (LACI), including Studio Colombia Performance Team and Studio 8 by LACI.
