Studio Colombia Performance Team

Waiver & Policies Agreement

1. Assumption of Risk & Liability Waiver

I understand that participation in dance rehearsals, performances, workshops, travel, and related activities with Studio Colombia Performance Team involves physical activity that carries inherent risks, including but not limited to muscle strains, falls, or other injuries.

I voluntarily assume full responsibility for any risk of injury, loss, or damage arising from participation. I hereby release and hold harmless Studio Colombia, Latino Arts & Cultural Institute (LACI), instructors, choreographers, staff, volunteers, partners, and venues from any and all claims or liability arising from participation, except in cases of gross negligence.

2. Medical Treatment Authorization

In the event of an injury or medical emergency, I authorize Studio Colombia staff to seek emergency medical treatment if necessary. I understand that I am responsible for any medical expenses incurred.

3. Code of Conduct

Dancers are expected to:

Attend rehearsals and performances on time and prepared

Treat instructors, fellow dancers, staff, and community partners with respect

Maintain a positive, collaborative, and professional attitude

Follow choreography, costuming, and performance guidelines

Disruptive, unsafe, or disrespectful behavior may result in dismissal from a rehearsal, performance, or the team.

4. Attendance & Commitment

Studio Colombia Performance Team is a collaborative ensemble. Consistent attendance at scheduled rehearsals and performances is expected. Excessive absences or last-minute cancellations may impact casting, performance eligibility, or future participation.

5. Costumes & Materials

Dancers may be required to purchase, borrow, or care for costumes or accessories. Dancers are responsible for returning borrowed items in good condition or replacing them if lost or damaged.

6. Photo & Media Release

I grant permission for Studio Colombia and LACI to photograph or record performances, rehearsals, or events for promotional, educational, or archival purposes (including social media, website, and reports). No compensation will be provided.

7. Acknowledgment & Agreement

I have read and understand this Waiver & Policies Agreement and agree to abide by all Studio Colombia Performance Team guidelines.
