Acknowledgment of Risks
I, the undersigned, acknowledge and agree that participation in dance classes, rehearsals, performances, and any related activities ("the Activities") at Fusion Dance Studio involves inherent risks. These risks include, but are not limited to, the possibility of:
Physical injuries, such as sprains, strains, fractures, and other musculoskeletal injuries.
Head injuries, concussions, and other neurological damage.
Injuries resulting from physical exertion, including but not limited to falls, slips, twists, and impacts with other participants or surfaces.
Injuries caused by equipment, props, or the physical condition of the premises.
Medical conditions that may arise or be exacerbated by strenuous physical activity.
Permanent disability, paralysis, or death.
I understand that these risks cannot be completely eliminated without jeopardizing the essential qualities of the Activities. I also understand that injuries may occur despite the best efforts of the instructors, staff, and management of Fusion Dance Studio to create a safe environment.
Assumption of Risk
I, the undersigned, knowingly and voluntarily assume all risks, both known and unknown, that may be associated with my (or my child’s) participation in the Activities. I freely accept full responsibility for any and all injuries, damages, or losses that may result from my (or my child’s) participation.
Waiver and Release of Liability
In consideration for being permitted to participate in the Activities, I hereby waive, release, and forever discharge Fusion Dance Studio, its owners, instructors, employees, agents, and representatives ("the Released Parties") from any and all claims, demands, liabilities, actions, or causes of action of any kind, whether known or unknown, which arise or may arise from my (or my child’s) participation in the Activities.
This waiver and release includes, but is not limited to, any claims for personal injury, property damage, or wrongful death, whether caused by the negligence of the Released Parties or otherwise.
Indemnification
I further agree to indemnify, defend, and hold harmless the Released Parties from and against any and all claims, losses, damages, or expenses, including legal fees, that may be incurred as a result of any claim or lawsuit brought by me, my family, or any third party arising out of my (or my child’s) participation in the Activities.
Medical Authorization
In the event of an emergency, I grant Fusion Dance Studio permission to seek medical attention for me or my child, and I assume all costs associated with such treatment. I agree that the Released Parties are not responsible for any medical expenses incurred as a result of my (or my child’s) participation.
