The undersigned certifies that she/he is the parent and/or legal guardian of the minor child above and grants permission for said minor child to participate in the classes at the Santa Barbara Festival Ballet. I further, for myself and/or child, agree to hold harmless the Santa Barbara Festival Ballet and Santa Barbara Dance Center, its offers, employees and agents, for any injuries that may result from said participation associated therewith. I further authorize any representative of the Santa Barbara Festival Ballet to obtain any emergency medical treatment they deem necessary for myself or for my child.

I grant to Santa Barbara Festival Ballet, its representatives and employees the right to take photographs of the above named dancer. I authorize Santa Barbara Festival Ballet, its assignees and transferees to copyright, use and publish the same in print and/or electronically. I agree that Santa Barbara Festival Ballet may use such photographs of the dancer with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and Web content.
