Online Class Transition Waiver
In the event that we are unable to continue with in-person dance classes due to circumstances such as COVID-19 or any government-declared state of emergency, all classes will transition to live online instruction via Zoom. Classes will continue to be held at each dancer’s regularly scheduled time with their assigned teacher, allowing students to maintain their training and continue preparation for the end-of-year recital.
All enrolled students will be automatically transitioned to the online format unless we receive a written notice of withdrawal from the program. This notice must be submitted via email prior to the transition.

General Liability Waiver
I/We fully understand and acknowledge that participation in dance classes, rehearsals, performances, and related activities with Cassiah’s Dance Company involves certain inherent risks, including but not limited to physical injury, accidents, slips, falls, or other incidents that may result in personal harm or property damage.
Despite all safety precautions and best practices taken by Cassiah’s Dance Company and its staff, I/we acknowledge that injuries and accidents can occur during physical activities such as dance. These risks may include, but are not limited to: muscle strains, sprains, broken bones, concussions, or other potentially serious injuries.

By signing this waiver, I/we (the student and parent/legal guardian) voluntarily assume full responsibility for all risks of personal injury, illness, loss, or property damage that may be sustained by the student while participating in any activity associated with Cassiah’s Dance Company, whether on or off studio premises.

I/we hereby release, waive, and hold harmless Cassiah’s Dance Company, its owners, directors, faculty, instructors, staff, assistants, volunteers, guest teachers/choreographers, and any facility used by the company from any and all claims, liabilities, demands, costs, expenses, or causes of action, now or in the future, related to injuries, damages, or losses that may occur during or as a result of participation in dance activities, including travel to and from any event or class.

This waiver extends to all locations where Cassiah’s Dance Company activities take place, including but not limited to: studio space, theatre venues, competition and convention locations, parking lots, outdoor events, and virtual/online sessions.

I/we understand and agree to:

Obey all class and facility rules as outlined by Cassiah’s Dance Company.
Conduct ourselves in a respectful and responsible manner.
Take full responsibility for any damage caused by our actions to any facility or equipment used.
Report any unsafe conditions or conduct to the Studio Director, teacher, or staff member immediately.
Acknowledge that Cassiah’s Dance Company is licensed, accredited, and insured, and that safety is a top priority at all times.
In signing this waiver, I/we affirm that the student is in good physical condition and has no medical condition that would prevent full participation in dance activities. If the dancer has any limitations, allergies, or medical conditions, they must be disclosed in writing upon registration.

I/we hereby grant Cassiah’s Dance Company permission to take and use photographs, video recordings, and digital images of the student named below, taken during classes, rehearsals, performances, competitions, and studio events, for the following lawful purposes:

- Studio marketing and promotional materials
- Social media platforms (e.g., Facebook, Instagram, YouTube)
- The studio website and newsletters
- Print materials (e.g., posters, brochures, programs)
- Local media coverage (if applicable)

I/we understand that:
These images may be used in both online and offline formats to represent the studio positively.
No personal information (such as full name or contact info) will be shared publicly without additional consent.
All media content is the property of Cassiah’s Dance Company and may be used in perpetuity unless written notice of withdrawal is submitted.
Participation in the program implies consent, unless explicitly denied by contacting our office.

At Cassiah's Dance Company, we are committed to protecting your privacy and safeguarding the personal information you share with us. This Privacy Policy outlines how we collect, use, and protect the information of our students, parents, and website visitors.

1. Information We Collect
We may collect the following types of personal information:
- Student Information: Name, date of birth, medical concerns (if disclosed), class enrollments.
- Parent/Guardian Information: Name, address, phone number, email address, emergency contacts.
- Payment Information: Billing address, payment history (note: we do not store full credit/debit card information if using third-party payment processors).
- Media: Photos and videos taken during classes, events, and performances (with consent).

2. How We Use Your Information
We use your information to:
- Register and manage student enrollment.
- Communicate with parents/guardians regarding class updates, billing, schedules, and studio news.
- Process payments and issue invoices.
- Promote studio events, such as recitals or showcases (with media releases when applicable).

3. Sharing of Information
We do not sell, rent, or trade your personal information. Information may be shared with third parties only in the following cases:
- With service providers (e.g., registration platforms, payment processors) to facilitate our services.
- With emergency responders if necessary for the safety or health of a student.
- With your consent (e.g., inclusion in recital programs, photos on social media, etc.).
- When required by law or legal process.

4. Media and Photography Policy
With your written consent, photos and videos of students may be used for promotional purposes, including on our website, social media, and printed materials. You may opt out at any time by notifying us in writing.

5. Data Security
We take reasonable precautions to protect your personal information. Access is limited to staff who need the information to perform their job duties. Digital data is stored securely through password-protected systems and trusted third-party platforms.

6. Your Rights and Choices
You have the right to:
- Access or correct your personal information.
- Withdraw consent for use of photos or communications.
- Request deletion of your data (subject to legal and operational obligations).

7. Changes to This Policy
We may update this Privacy Policy from time to time. Any changes will be posted in your parent portal and effective immediately upon posting.

As part of the registration for the Cassiah's Dance Company 2025–2026 dance season, all parents/guardians must acknowledge and agree to the following recital and costume policy:

Recital Participation:
All dance classes at Cassiah's Dance Company are designed to prepare students for participation in our annual End-of-Year Recital, scheduled for June 2026. Dancer participation in the recital is an integral part of the program and curriculum.

Costume Rental Fees:
A costume rental fee will be required for each class your dancer is enrolled in. Each costume corresponds to one class and will be used for the recital. This rental fee covers the temporary use of costumes and ensures consistency and quality in appearance for all performers.
Example: If your child is enrolled in 3 classes, you will be responsible for 3 separate costume rental fees.

Recital Ticket Requirement:
Parents/guardians and other guests who wish to attend the recital will be required to purchase tickets. Ticket sales help offset the costs associated with the venue, staffing, and production of the recital.

Non-Refundable Fees:
All costume rental fees are non refundable after April 1 2026

Agreement to Terms:
By completing registration, I acknowledge that I have read and understand the above policies. I agree to pay the applicable costume rental fees and purchase recital tickets as required. I understand that participation in the recital is expected as part of the dance curriculum.

All payments made to Cassiah’s Dance Company are non-refundable. This includes, but is not limited to:
- Registration fees
- Tuition payments
- Competition fees
- Workshop/masterclass fees
- Choreography fees
- Costume payments
- Merchandise purchases

In the event of a withdrawal, cancellation, or missed class, no refunds will be issued. However, a credit may be applied to your account to be used toward future classes, merchandise, or other eligible studio expenses, at the discretion of the studio director.

Recital Costume Fees:
All costume payments are non-refundable after April 1st, 2026, regardless of withdrawal, absence, or dismissal.

Termination of Enrolment
At Cassiah’s Dance Company, we are committed to maintaining a positive, respectful, and supportive environment for all dancers, staff, and families. In rare cases, when a dancer or parent’s behavior negatively impacts the learning environment or the well-being of others, termination of enrolment may be necessary.
While every effort will be made to address and resolve concerns through communication and corrective steps, Cassiah’s Dance Company reserves the right to dismiss a dancer from the program if the situation warrants.

Grounds for Termination (including but not limited to):
- Disruptive or unsafe behavior by a student or parent.
- Verbal or physical abuse toward fellow students, staff, or property.
- Ongoing negativity or disrespect toward Cassiah’s Dance Company, instructors, or other members—this includes inappropriate conversations, gossip, or social media conduct.
- Persistent lack of focus, effort, or poor attitude during classes, rehearsals, or performances.
- Failure to adhere to studio policies, including respect for authority and team values.
- Inability of the studio to meet the student’s developmental, behavioral, or learning needs.
We hold our dancers and their families to high standards of conduct. Company members are expected to demonstrate courtesy, respect, and dedication in every class, rehearsal, and performance.

If a student will no longer be attending classes at Cassiah’s Dance Company, it is the responsibility of the Parent/Guardian to officially withdraw the dancer by completing a Withdrawal Form.
Until this form is submitted and processed, the student will be considered active in our program, regardless of attendance, and all applicable fees will continue to be charged.

Withdrawal Requirements:
A Withdrawal Form must be submitted to our office 30 days prior to the intended withdrawal date.
Tuition and applicable late fees will continue to accrue until 30 days after the withdrawal notice and form are received.
Verbal communication (phone, email, or text) is not accepted as formal notice of withdrawal.

Financial Responsibility:
Monthly tuition will continue to be charged until the end of the current payment term (May 2026) unless the Withdrawal Form and 30-day notice are provided.
Full-year payments or prepaid monthly tuition are non-refundable. A credit may be applied to your account for future classes or merchandise, at the discretion of the director.
Costume fees are non-refundable after April 1st, 2026, regardless of withdrawal or absence.

Emergency Closure Clause:
In the event of an emergency that requires the studio to close (e.g. government-mandated lockdown), all classes will transition to our online training program via Zoom or Google Meet. Regular attendance and participation will still be expected. The No Refund Policy remains in effect under these circumstances.
