2026–2027 Universal Facility & Activity Waiver

This is a binding legal agreement. Please read carefully before accepting.

1. ASSUMPTION OF RISK
Participation in activities at Vlad’s Dance Company (“VDC”) includes, but is not limited to:
• Dance technique classes
• Rehearsals
• Competitive training
• Camps and intensives
• Workshops and conventions
• Private lessons
• Performances and recitals
• Drop-in classes
• Travel to and from events
• Use of studio equipment and facilities
Dance training involves inherent physical risks including, but not limited to:
• Musculoskeletal injuries
• Sprains, strains, fractures, or dislocations
• Slips, falls, or collisions
• Physical contact with other participants
• Use of props or equipment
• Fatigue and physical exertion
I acknowledge that participation is voluntary and that these risks cannot be completely eliminated.

2. RELEASE OF LIABILITY
In consideration of being permitted to participate in activities at VDC, I (and/or as parent/guardian of the participant) release and discharge Vlad’s Dance Company, its owners, directors, instructors, employees, contractors, and representatives from any and all claims, demands, damages, actions, or causes of action arising from participation in studio activities, including claims arising from the ordinary negligence of Vlad’s Dance Company, except where prohibited by the laws of the Province of Ontario.

3. MEDICAL AUTHORIZATION
I authorize VDC to obtain emergency medical treatment for the participant if deemed necessary.
I understand that I am financially responsible for any medical care required.

4. HEALTH & ILLNESS
Participants must not attend the studio if experiencing symptoms of illness.
VDC reserves the right to refuse participation if a participant appears unwell.
In the event of a government-mandated closure, programming may transition to virtual instruction. No refunds or credits will be issued for mandated closures.

5. PROPERTY DISCLAIMER
VDC is not responsible for lost, stolen, or damaged personal property brought onto the premises.

6. MEDIA RELEASE
I grant permission for photographs and/or video recordings of the participant to be used for studio promotional purposes including website, social media, advertising, and printed materials.

7. STUDIO CONDUCT
All participants must comply with studio policies, including:
• Dress code requirements
• Nut-free policy
• No food or drink in studios (water permitted)
• Respectful behavior toward staff and peers
• No bullying, misconduct, or unsafe behavior
VDC reserves the right to suspend or remove any participant for misconduct, safety concerns, or policy violations.

8. FINANCIAL ENFORCEMENT
VDC reserves the right to suspend participation in any class, rehearsal, performance, competition, workshop, or event if accounts remain unpaid beyond established due dates.

9. DROP-IN & NON-SEASONAL PARTICIPATION
This waiver applies whether the participant is:
• Enrolled in a full-season program
• Attending a summer program
• Participating in a competitive program
• Taking a trial class
• Attending a drop-in class
• Participating in a workshop or special event
This waiver applies to all participation at VDC during the validity period regardless of enrollment status.

10. JURISDICTION
This agreement shall be governed by and interpreted in accordance with the laws of the Province of Ontario.

11. SEVERABILITY
If any portion of this agreement is deemed invalid or unenforceable, the remaining provisions shall remain in full force and effect.

12. ACKNOWLEDGMENT
By accepting below, I acknowledge that:
• I have read and understand this agreement
• I am accepting this agreement voluntarily
• This agreement is binding upon myself and, if applicable, my child
