Andrea’s Dance Academy VT TERMS & CONDITIONS Please sign the bottom agreeing to our policies and terms: ______I understand that all monthly tuition fees are due on the 1st class of every month. ______I understand that if my payment is not made 7 days after the first class, there will be a $12 fee added to my account. ______I understand that all fees are non-refundable. ______I understand that if I have an outstanding balance after 30 days, my child will not be able to attend until my balance is current. _____I understand that my child will have to attend ALL dance recitals and rehearsals _____I understand that ALL private lesson/use of the studio are to be approved by Ms. Andrea. _____I understand I'm responsible for purchasing my dancers' competition, recreational costumes and shoes for each dance. _____I understand the attire & shoe requirements. _____I understand the attendance policy. _____I understand all logos are owned by Andrea’s Dance Academy VT and cannot be used to make your own gear. ____ I have understood bullying will not be tolerated under any circumstances.
Andrea’s Dance Academy Release and Waiver of Liability I understand that risk of injury is inherent in any physical activity and I, on behalf of myself and my child, knowingly and voluntarily accept that risk. I, the undersigned, hereby waive and release Andrea’s Dance Academy from any and all claims or damages of any kind arising out of my child’s participation in the dance program. I, the undersigned, do hereby authorize Andrea’s Dance Academy or her designated teacher to obtain medical treatment for my said child in emergency situations where I cannot be reached in time to authorize the treating physician to provide such emergency medical services.
