1. Assumption of Risk and Waiver of Liability
I understand that participation in Centerstage classes, rehearsals, and performances
involves physical activity and the risk of injury. I, or on behalf of my child, voluntarily
assume all risks associated with such participation, including but not limited to slips,
falls, collisions, or other accidental injuries that may occur on or around St. George
Musical Theater or Centerstage property.
2. Medical Treatment Consent
In the event of a medical emergency, I authorize Centerstage staff to obtain emergency
medical treatment for my child. I agree to be financially responsible for any costs
incurred as a result of such treatment.
3. Photo and Media Release
I grant permission for Centerstage to use photographs, video, or audio recordings of the
participant for promotional purposes, including social media, websites, programs, and
printed materials.
4. Communicable Disease Acknowledgment
I understand that participation may expose my child to contagious illnesses, including
COVID-19. Despite precautions taken, I acknowledge this risk and release Centerstage
and SGMT from liability should such exposure occur during participation.
5. Code of Conduct and Policy Agreement
To ensure a safe, respectful, and positive environment for all students, staff, and families, we ask that all participants review and agree to the following policies:
Respect & Conduct
Kindness is required. Bullying, teasing, or exclusion of any kind will not be tolerated.
Students must treat instructors, classmates, and staff with respect at all times.
Swearing, rude language, or inappropriate behavior is not allowed on the premises.
Phones & Devices
Cell phones must be silenced and put away during class unless given permission.
Facility Respect
Students must respect the studio space and clean up after themselves.
No food or drink (except water in a closed container) is allowed in the studios.
Students may not deface or vandalize the facilities in any way. Vandalism will result in suspension from the camp/production/class and additional fees to cover the damages. This includes sets or props from other productions.
Participation & Focus
Students must arrive on time, dressed appropriately, and ready to participate.
Disruptive behavior may result in removal from class.
Designated Rehearsal Space
It is crucial that all students stay in designated rehearsal/classroom spaces at all times. NO wandering around the building is allowed. Often other SGMT productions or companies are holding rehearsals or events.
Refund Policy
Refunds are not given for missed classes. Make-up options are not guaranteed.
Electronic Devices
Students are asked not to bring tablets, phones, cameras, laptops or any other electronic device which may become damaged, lost or stolen. Centerstage staff and volunteers are not liable for any damaged, lost or stolen items which students/performers elect to bring to rehearsals or class.
Other Prohibited Items
No gum, toys, or fidget items unless approved.
No weapons, drugs, alcohol, or vaping devices are permitted on SGMT property.
6. Utah-Specific Indemnification Clause
I agree to indemnify, defend, and hold harmless Centerstage, St. George Musical Theater, and their staff, volunteers, directors and agents from any claims, demands, losses, or liabilities (including attorney’s fees) suffered by me or my child, arising from my or my child’s participation in the programming Centerstage activities or caused by their actions, negligence, or failure to follow rules—except in cases of gross negligence or intentional wrongdoing by Centerstage or SGMT.
7, This Agreement indemnification clause is subject to the laws of the State of Utah. is intended to be enforceable to the fullest extent permitted under Utah law.
Drop-off & Pick-up Policy
Centerstage is not responsible for students before or after their scheduled class, rehearsal, or camp times. Students should be dropped off no more than 10 minutes before their scheduled activity and must be picked up promptly afterward. Only individuals listed as authorized on the registration form may pick up a student. If a student is not picked up within 15 minutes of their scheduled end time, a late pick-up fee may be charged. Repeated late pick-ups may result in removal from the program.
