As the parent or legal guardian, I authorize Cleveland City Ballet staff and representatives to provide basic first aid to my child and, if necessary, secure emergency medical care, including transportation to a medical facility. I understand that every reasonable effort will be made to contact me or the designated emergency contact in the event of an emergency. I hereby release and hold harmless Cleveland City Ballet, its staff, and representatives from any liability, claims, or demands arising from my child’s participation in this program or any related activities.
By enrolling my child, I give permission for their photograph to be taken and used in Cleveland City Ballet promotional materials, including but not limited to brochures, the website, and other publications. I understand that my child’s name will not be used or disclosed in any of these materials.
