I/we realize that participation in dance classes and activities could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by Gravity Dance Studio.

I/we agree to release and hold harmless Gravity Dance Studio, including its teachers, dancers, staff members, and facilities used by both entities from any cause of action, claims, or demands now and in the future. I/we will not hold Gravity Dance Studio liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes. Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by Gravity Dance Studio.

I understand that Gravity Dance Studio is a licensed and insured business. In the event that I/we should observe any unsafe conduct or conditions before, during or after my/our classes, I/we agree to report the unsafe conduct or conditions to the Owner, Director, Instructor or Staff member as soon as possible.

Consent and Release
I, the undersigned, hereby grant permission to Gravity Dance Studio to take and use photographs and/or video recordings of me and/or my child for educational, promotional, and marketing purposes. Voluntary Participation: My participation and/or my child’s participation is voluntary, and I am not entitled to any compensation for the use of these images.
Ownership and Use: Gravity Dance Studio shall own all rights to the images and recordings, and may edit, alter, copy, exhibit, publish, or distribute them for lawful purposes.

No Expiration: This consent is granted for an indefinite period and does not require prior notification for each use.

Recreational Instruction Contract

Welcome to the Gravity Dance Studio. We are so glad you have chosen our studio to help mold your dancer! We look forward to getting to know your child and working to bring him/her into the joy that being a dancer can bring! Please read and agree to the below:
Attendance
Dancers are required to attend all classes and dress rehearsals for the end of the season recital. Multiple absences will adversely affect the individual dancer and their growth. Each dancer is allowed up to three (3) unexcused absences from team classes and rehearsals.
Please let us know if the dancer will miss class in advance.
The following are considered “excused” absences:
• Death in the family or family emergency
• Scheduled school programs or performances
• Wedding or religious dedication of you or a member of your family
• Illness
Dancers are asked to watch class/rehearsal in case of injury in order to keep up with changes to choreography. Punctuality is just as important as attendance so please be on time and prepared for class. Dance requires consistent ongoing training and dancers who excessively miss classes will not be able to perform at the same level as those who have attended all classes regularly.
Time & Financial Commitment
Dancers will be required to wear specific shoes (ballet slipper/pointe shoes, jazz shoes, tap shoes, etc.) as well as tights and leotards. Fees will include tuition, costumes, recital(s) facility fees, etc. For the specific financial commitment information please read through the “Fees” portion of this contract below.
Conduct
All dancers are held to the highest standards of conduct. Communicating negatively about anyone in or outside of our studio will not be tolerated. We ask that each student show courtesy and respect to all instructors and other students. Any student who exhibits repeated negative or disrespectful behavior will be asked to leave the competition team. Our dancers are always expected to be respectful, supportive, encouraging and are positive in spirit, attitude and behavior.
Dress Code
All Dance members must wear appropriate dancewear to every class.
The dress code is as follows:
Recreational Classes/Technique - Pink tights, leotard, appropriate shoes, hair back and off of face/neck (unless otherwise instructed by teacher). Bike shorts/yoga pants, short to knee length dance skirts are permitted (except in Ballet).

Fees: All fees are due by the 1st of each month or the first day of class thereafter.
Tuition: _____$65/mo for 1 class

______$117/mo for 2 classes


Costume Fee: $80 per costume (average estimate only)

Christmas Show Fee: $15
Tights for Christmas Show: $11
Recital Fee: $50
Tights for Recital: $11
Registration Fee: $25 (Due at Time of Registration)

Dance Shoes/ Class Attire (prices will vary) Can purchase from the store of choice or can purchase from the studio. **Please note that Dance shoes for performances must be a particular brand so that all dancers match.
