In consideration of participating in all types of activities at Vive la Danse Dance Academy Inc., including but not limited to dance, movement, stretching, exercise, aerial silks, aerial hoops, trapeze, acrobatics, cheer, hip hop, break dancing, gymnastic activities, ballroom, camps, classes, examinations, private lessons, workshops, conventions, choreography, strength training, flexibility training, photo shoots, video shoots, parades, competitions, festivals, team-building events, promotional events, streaming events, performances, theory training, being fitted for and wearing costumes, wearing makeup, having hair arranged, grooming, and travelling to and from locations where the aforementioned occur (collectively the "activities"), I represent that I understand the nature of this activity and that I am qualified, in good health, and in proper physical condition to participate in such activity. I acknowledge that if I believe conditions are unsafe, I will immediately discontinue participation in the activity. I acknowledge that I am voluntarily participating in this activity. I am fully aware that all activities pose dangers and inherent risks (including serious injury or death), that no amount of care, caution, instruction, or expertise can eliminate. I understand the scope, nature and extent
of the risks involved and voluntarily and freely choose to assume any and all such risks. I fully understand that this activity
involves risks of serious bodily injury, including permanent disability, paralysis and death, which may be caused by my own
actions, or inactions, those of others participating in the event, the conditions in which the event takes place, or the
negligence of the “releasees” named below; and that there may be other risks either not known to me or not readily
foreseeable at this time; and I fully accept and assume all such risks and all responsibility for losses, cost, and damages I
incur as a result of my participation in the activity. I hereby release, discharge, and covenant not to sue Vive la Danse Dance Academy Inc., its respective administrators, owners, directors, volunteers, contractors, and employees, other participants, and, if applicable,
owners and lessors of premises on which the activity takes place, (each considered one of the “RELEASEES” herein) from all
liability, claims, demands, losses, or damages on my account caused or alleged to be caused in whole or in part by the
negligence of the “Releasees” or otherwise, including negligent rescue operations and future agree that if, despite this
release, waiver of liability, and assumption of risk I, or anyone on my behalf, makes a claim against any of the Releasees, I
will indemnify, save, and hold harmless each of the Releasees from any loss, liability, damage, or cost, which any may incur
as a result of such claim. Parents and/or guardians are solely responsible for getting their minor to and from the facility
safely and holds Vive la Danse Dance Academy Inc. harmless of any events that may occur leaving or coming into the facility.
I have read the RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT, understand that
I have given up substantial rights by signing it and have signed it freely and without any inducement or assurance of any
nature and intend it to be a complete and unconditional release of all liability to the greatest extent allowed by law and
agree that if any portion of this agreement is held to be invalid the balance, notwithstanding, shall continue in full force
and effect. I acknowledge that I have been advised to obtain independent legal advice concerning the ramifications of
signing this document.
I, the minor’s parent, and/or legal guardian, understand the nature of the above referenced activities and the Minor’s
experience and capabilities and believe the minor to be qualified to participate in such activity. I hereby Release, discharge,
covenant not to sue and AGREE TO INDEMNIFY AND SAVE AND HOLD HARMLESS Vive la Danse Dance Academy Inc. and each of the
Releasees from all liability, claims demands, losses or damages on the minor’s account caused or alleged to have been
caused in whole or in part by the negligence of the Releasees or otherwise, including negligent rescue operations, and
further agree that if, despite this release, I, the minor, or anyone on the minor’s behalf makes a claim against any of the
above Releasees, I WILL INDEMNIFY, SAVE AND HOLD HARMLESS each of the Releasees from any litigation expenses,
attorney fees, loss liability, damage, or cost of any Releasee may incur as the result of any such claim. I acknowledge that I
have been advised to obtain independent legal advice concerning the ramifications of signing this document.

Vive la Danse Photo/Video Release

By signing the release form, I authorize Vive la Danse Dance Academy Inc., to use the following personal information:
(1) My picture – including photographic, motion picture, and electronic (video) images.
(2) My voice – including sound and video recordings.
I hereby grant to Vive la Danse Dance Academy Inc, its subsidiaries, licensees, successors and assigns, the right
to use, publish, and reproduce, for all purposes, my name, pictures of me in film or electronic (video)
form, sound and video recordings of my voice, and printed and electronic copy of the information
described in sections (1) and (2) above in any and all media including, without limitation, cable and
broadcast television and the Internet, and for exhibition, distribution, promotion, advertising, sale,
press conferences, meetings, hearings, educational conferences and in brochures and other print
media. This permission extends to all languages, media, formats and markets now known or hereafter
devised. This permission shall continue forever unless I revoke the permission in writing.
I further grant Vive la Danse Dance Academy Inc. all right, title, and interest that I may have in all finished pictures,
negatives, reproductions, and copies of the original print, and further grant Vive la Danse Dance Academy Inc. the right to give, sell, transfer, and exhibit the print in copies or facsimiles thereof, for marketing,
communications, or advertising purposes, as it deems fit.
I hereby waive the right to receive any payment for signing this release and waive the right to
receive any payment for Vive la Danse Dance Academy Inc.’s use of any of the material described above for any
of the purposes authorized by this release. I also waive any right to inspect or approve finished
photographs, audio, video, multimedia, or advertising recordings and copy or printed matter or
computer generated scanned image and other electronic media that may be used in conjunction
wherewith or to approve the eventual use that it might be applied.

I acknowledge that I have read the foregoing and I fully understand the contents.

I hereby certify that I am the parent or guardian of
who is under the age of 18 years, to whom this release applies and that I have the legal authority to
execute this release. I approve the foregoing and agree that we both shall be bound thereby.
