Acknowledgment & Agreement

By signing below, I confirm that I have read, understood, and agreed to Studio650’s Liability Waiver, Media Release, and Financial Agreement. I acknowledge that these documents outline important policies and expectations, and I agree to abide by all terms set forth.

1. Payment Authorization
By signing this agreement, I authorize Studio650 to automatically charge the payment method I provide for all tuition and related fees. I confirm that I am the authorized user of the account and have the legal right to approve these transactions.

2. Billing Schedule
Tuition will be processed on the 1st of each month.
I understand that it is my responsibility to maintain sufficient funds to cover each scheduled payment.
Failed transactions may be subject to additional processing fees.

3. Autopay Cancellation
I understand that I must cancel my autopay on my own through my online account prior to the 1st of the month to avoid being charged.
Studio650 will not issue refunds for payments processed due to failure to cancel before the billing date.

4. Refunds & Delinquent Accounts
All tuition and associated fees are final and non-refundable.

Studio650 reserves the right to:
Charge any valid card or account on file for past-due balances.
Suspend or discontinue services for accounts with unpaid balances.

By signing below, I acknowledge that I have read, understand, and agree to the terms outlined in this financial agreement. I understand that this authorization will remain in effect until I submit a cancellation request in accordance with the terms stated above.

Studio650 General Release, Liability Waiver, and Media Policy Agreement

Participant Agreement
As the parent or legal guardian of a minor child enrolling in dance and dance-related programs with Studio650, I acknowledge and agree to the following:

I understand that participation in dance classes, rehearsals, workshops, intensives, performances, competitions, and other Studio650 activities involves inherent physical risks. I certify that my child is in good health and has no physical limitations that would preclude safe participation, or I have informed Studio650 of such limitations.
I voluntarily assume full responsibility for any risks of injury or harm to my child or myself that may result from participation in Studio650 activities, whether held on-site or off-site, unless caused by negligence or willful misconduct by Studio650 or its representatives.
Locations and Transportation
I understand that classes and activities may occur at multiple locations, including Menlo-Atherton High School and the Studio650 facilities at:
1923 Menalto Ave, Menlo Park, CA
3536 Alameda de las Pulgas, Menlo Park, CA
440 Kipling St, Palo Alto, CA
I acknowledge that Studio650 staff or representatives may occasionally transport students between locations or to events (competitions, conventions, performances) as a courtesy. I understand that Studio650 is not liable for injuries or damages sustained during transportation unless arising from negligence or willful misconduct.

Liability Waiver
I, on behalf of myself, my child, and our respective heirs, assigns, and legal representatives, release, indemnify, and hold harmless Studio650 and its owners, staff, volunteers, instructors, contractors, and affiliates from any claims, damages, injuries, or losses resulting from participation in Studio650 activities, except in cases of proven negligence or willful misconduct.

Medical Authorization
I authorize Studio650 and its representatives to seek emergency medical and/or dental treatment for my child in the event of an injury or medical emergency when I cannot be reached.
I authorize medical personnel to administer necessary medical care, including tests, medication, or emergency procedures as deemed appropriate.
I accept full financial responsibility for any resulting medical treatment or expenses.
I agree to provide Studio650 with updated medical information, including life-threatening allergies or other medical conditions requiring monitoring or accommodations.
Important: Studio650 does not administer medications (including aspirin) and expects parents to make arrangements in advance for any necessary medications or interventions.

Media Release
I grant Studio650 permission to record and use photographs, video, and other media of my child participating in dance-related activities for promotional and marketing purposes, including but not limited to websites, social media, advertising, and printed materials. I understand that these images may be used without compensation and remain the property of Studio650.

General Acknowledgments
I understand that dance is a physically demanding activity. Students may decline participation in any activity they are uncomfortable with.
I agree to communicate any injuries, limitations, or concerns with Studio650 staff in a timely manner.
I understand that while Studio650 prioritizes safety, the studio is not liable for lost, stolen, or damaged personal items.

Final Consent
By signing below, I confirm that I have read, understood, and agree to all terms outlined in this agreement. If the participant is under the age of 18, I have explained this waiver to them and agree on their behalf.
