I have chosen to enroll my child(ren) for dance instruction at Glow Up Dance Studio. I realize by signing I am agreeing to all terms listed in this handbook. I know that despite best efforts, dance is a sport and there may be injuries that could occur. I acknowledge the staff at Glow Up Dance Studio are not responsible for injuries and will always put the safety of my child(ren) above all else. I will not hold Glow Up Dance Studio liable for any injuries, accidents, death, or illnesses that may occur on the premises. I also agree to be financially responsible for any damages to Glow Up property or property of Linda’s Olympian Dance Studio caused by my child(ren) or myself. I give permission for the staff at Glow Up to seek medical help in the event of an emergency. I agree that all of my contact information, my child’s medical information, and emergency contact information is accurate and up to date. I agree not to use the Glow Up Dance Studio logo without prior permission from its owners. I agree that all choreography is property of Glow Up Dance Studio and may not be sold or distributed. I know that Glow Up Dance studio is not responsible for lost or stolen items. I understand the staff at Glow Up are people and their personal lives are private, questions about staff absences will not be permitted. I know that I will not receive a refund for missed classes, this includes absences, holidays and cancellations due to inclement weather. I understand the need to make payments on time, I agree to pay any late fees I may incur, and I give permission to run my AutoPay card on file for tuition and any overdue charges of more than 2 weeks. I know Glow Up Dance Studio will not sell or distribute my personal information to any other party.

I, the undersigned parent or legal guardian of the participant, hereby acknowledge and agree to the following terms and conditions in consideration for participation in any classes, rehearsals, performances, or events (collectively referred to as “Activities”) hosted by Glow Up Dance Studio (“the Studio”).
Assumption of Risk & Release of Liability
I understand and acknowledge that participation in dance and related activities involves inherent risks, including but not limited to physical injury. I hereby voluntarily assume all risks associated with participation in any Studio Activities, whether occurring on or off the premises.
I hereby waive, release, and discharge Glow Up Dance Studio, its owners, instructors, staff, contractors, and representatives from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or expense that may be sustained by the participant or myself while participating in Studio Activities, whether caused by the negligence of the Studio or otherwise.
Financial Responsibility
I understand and agree that Glow Up Dance Studio shall not be held financially responsible for any medical costs, treatment, or other expenses incurred as a result of injury or accident during participation in any Studio Activity, on the premises, or at any event sponsored or affiliated with the Studio.
Rules and Handbook Acknowledgement
I acknowledge that I have received, read, and understood the Glow Up Dance Studio Parent and Student Handbook. I agree to comply with all rules, policies, and procedures outlined in the Handbook, and I understand that failure to follow these guidelines may result in suspension or termination of participation without refund.
