Assumption of Risk and Release of Liability:

I, [Parent/Guardian Name] (if participant is under 18) or [Participant Name], acknowledge that participation in the Florida Dance Gym LLC involves physical activity and carries inherent risks of injury, including but not limited to muscle strains, sprains, fractures, and other physical injuries. I voluntarily assume all such risks. I release and discharge Florida Dance Gym LLC, its owners, instructors, employees, and volunteers from any and all liability for any injury, loss, or damage arising from my/my child's participation, whether caused by negligence or otherwise.

No Refund/No Chargeback Policy:

I understand and agree that all fees paid for the Florida Dance Gym LLC are non-refundable. Once payment is made, no refunds will be issued for any reason, including but not limited to absence, withdrawal, or cancellation. I further agree that I will not initiate any chargebacks with my credit card company or bank for any fees paid to Florida Dance Gym LLC for this program. Initiating a chargeback will result in the participant being removed from the program.

Cancellation Policy (COVID, Storms, and Other Circumstances):

In the event that the Florida Dance Gym LLC is canceled or postponed due to COVID-19 related restrictions, severe weather conditions (including but not limited to hurricanes, tropical storms), or other unforeseen circumstances, Florida Dance Gym LLC will make reasonable efforts to reschedule the program or provide alternative options, such as online classes or credit towards future sessions. However, no refunds will be issued.

Medical Information and Release:

I certify that I/my child am/is physically fit to participate in dance and acrobatic classes at Florida Dance Gym LLC. I agree to notify Florida Dance Gym LLC of any medical conditions or injuries that may affect my/my child's participation. I grant permission to Florida Dance Gym LLC to seek emergency medical treatment for me/my child if necessary.

Photo/Video Release:

I grant permission to Florida Dance Gym LLC to use photographs and/or videos of me/my child for promotional purposes, including but not limited to website, social media, and marketing materials.
