To create an account with The PEAK School of Dance, you must acknowledge and agree to the following:

You must be 18 years or older to register for an account.

You agree to provide accurate and up-to-date information for your child’s enrollment.

You have read and agree to The PEAK School of Dance Privacy Policy, No Refund Policy, and Waiver.

You understand that all payments made are non-refundable and non-transferable.

You acknowledge that all communication regarding your child’s classes, billing, and studio updates will be sent via the contact information provided.

The PEAK School of Dance Privacy Policy, No Refund Policy & Waiver

By completing your online registration for classes at The PEAK School of Dance, you acknowledge and agree to the following Privacy Policy, No Refund Policy, and Waiver regarding the collection, use, and protection of your personal information.

1. Collection of Information

When registering for classes, The PEAK School of Dance collects personal information, including but not limited to:

Parent/guardian name(s)

Student name(s) and birthdate(s)

Contact information (address, phone number, email)

Emergency contact details

Payment information for tuition and fees

Medical or allergy information (if applicable)

2. Use of Information

Your information is used solely for purposes related to The PEAK School of Dance, including but not limited to:

Class registration and scheduling

Tuition billing and payment processing

Emergency notifications

Studio communications (newsletters, updates, event details, etc.)

Internal record-keeping

The PEAK School of Dance will never sell, trade, or share your personal information with third parties for marketing or other commercial purposes.

3. Protection of Information

We take reasonable precautions to ensure that your personal information is secure. Digital records are stored with password protection, and any physical records are kept in a secured location. While we strive to protect your data, we cannot guarantee absolute security in the case of unforeseen circumstances (e.g., cyber breaches).

4. Photo & Video Release

By agreeing to this waiver, you grant The PEAK School of Dance permission to use photos and videos of your child(ren) taken during classes, performances, or studio events for promotional purposes, including but not limited to:

Social media

Studio website

Printed materials

Advertising and marketing

If you wish to opt out of photo/video usage, you must notify The PEAK School of Dance in writing before your child begins classes.

5. No Refund Policy

By registering for classes at The PEAK School of Dance, you acknowledge and agree to the following no refund policy:

Tuition, registration fees, costume fees, and recital fees are non-refundable and non-transferable.

If a student withdraws from a class, no refunds or credits will be given for missed classes, absences, or unused tuition.

Tuition is based on enrollment, not attendance. Missed classes may be made up in another age-appropriate class within the same session but will not result in a tuition refund or credit.

If The PEAK School of Dance must close due to circumstances beyond our control (e.g., weather, health mandates, or emergencies), classes will be rescheduled, moved to an online format, or an alternative solution will be offered. Refunds will not be issued.

By completing your registration, you agree to this No Refund Policy and understand that all payments are final.

6. Third-Party Services

The PEAK School of Dance may use third-party software for class registration, payment processing, and communication (e.g., email services, scheduling apps). These platforms have their own privacy policies, and The PEAK School of Dance is not responsible for data security within these third-party services.

7. Parent & Guardian Responsibility

By submitting this electronic waiver, you acknowledge that:

You have read and understand The PEAK School of Dance Privacy Policy, No Refund Policy, and Waiver.

You consent to the collection and use of your personal information as outlined above.

You agree to The PEAK School of Dance’s No Refund Policy and understand that all payments made are non-refundable and non-transferable.

You understand that it is your responsibility to update The PEAK School of Dance with any changes to your contact or medical information.

By checking the box and submitting this form electronically, you agree to the terms outlined in this Privacy Policy, No Refund Policy & Waiver.

By selecting the "I have read and agree" checkbox, I am signing this agreement electronically. I agree this electronic signature is the legal equivalent of my manual signature on this agreement.

By registering my child(ren) with the The Peak School of Dance, I agree to participate (or allow my child(ren) and family members to participate) in the The Peak School of Dance, and hereby release The Peak School of Dance, its directors, officers, agents, coaches, and employees from liability for any injury that might occur to myself (or to my child(ren) and family members) while participating in the The Peak School of Dance program.

I agree to indemnify and hold harmless the above mentioned organizations and/or individuals, their agents and/or employees, against any and all liability for personal injury, including injuries resulting in death to me, my child(ren) and/or other family members, or damage to my property, the property to my child(ren) and/or other family members, or both, while I (or my child(ren) or family members) participating in the The Peak School of Dance program and other classes.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connectioon with my child(ren)'s attendance at The PEAK School of Dance or participation in PEAK programming ("Claims"). On my behalf, and on behalf of my child(ren), I hereby release, covenant not to sue, discharge, and hold harmless The PEAK School of Dance, it's employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto.

I certify that I am the parent or legal guardian for my child(ren). I hereby give my permission for any supervisor, coach or other team administrator associated with the The Peak School of Dance to seek and give appropriate medical attention for our child(ren) in the event of an accident, injury, illness. I will be responsible for any and all costs associated with any necessary medical attention and/or treatment.

I hereby waive, release and forever discharge The Peak School of Dance from all rights and claims for damages, injury, loss to person or property which may be sustained or occur during participation in The Peak School of Dance activities, whether or not damages or loss is due to negligence. I hereby acknowledge that my children is (are) physically fit and capable of participation in all activities.
