Studio Information and Policies Waiver

I have read and acknowledged the policies in The PEAK School of Dance General Information Packet found in my welcome email or www.thepeakschoolofdance.com which includes their policies and procedures and will take the responsibility to carefully read and follow the rules and policies therein. I understand that The PEAK School of Dance does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, weather, etc. I also understand that anytime my card is charged through this system I will incur a 3.9% processing fee. I further understand that there are specific risks of physical or property damages, losses, or injury that may result from my or my own child's participation with The PEAK School of Dance, and I voluntarily assume the risks associated with such participation.
