1. Acknowledgement of risks - I, the undersigned participant (or parent/legal guardian if the participant is under 18), acknowledge that participation in acrobatic activities involves inherent risks, including but not limited to:
- Physical injuries such as sprains, fractures or concussions
- Muscle strain or overexertion
- Risks associated with falls or equipment failure.
I voluntarily choose to participate and assume all risks associated with acrobatics.

2. Waiver and Release of Liability - In consideration of being allowed to participate at Level Up Acro, I hereby release, waive, and discharge Level Up Acro, its owners, staff, coaches, volunteers, and affiliates from all liability, claims, demands, or causes of action related to injury, disability, death, or property damage arising from participation in the program.

3. Medical Treatment Authorization - I authorize Level Up Acro to provide first aid and, if necessary, seek medical treatment on my behalf in case of emergency. I understand that I am responsible for all medical expenses incurred.

4. Code of Conduct and Safety Agreement - I agree to follow all safety instructions and respect te rules set by the program staff. Failure to adhere to these rules may result in removal from the program without a refund.

5. Photo and Video Release - I grant permission to Level Up Acro to use photos/videos of my child for promotional purposes. If you do not agree, please email elevelupacro@gmail.com and state that you do not.

6. Acknowledgement and Signature - I have read this waiver, understand its terms, and agree voluntarily.
