Acknowledgement of Risks -
I, the undersigned participant (or parent/legal guardian if the participant is under 18), acknowledge that participation in acrobatic activities involves inherent risks, including but not limited to:
- Physical injuries such as sprains, fractures or concussions
- Muscle strain or overexertion
- Risks associated with falls or equipment failure.
I voluntarily choose to participate and assume all risks associated with acrobatics.
Waiver and Release of Liability
In consideration of being allowed to participate at Level Up Acro, I hereby release, waive, and discharge Level Up Acro, its owners, staff, coaches, volunteers, and affiliates from all liability, claims, demands, or causes of action related to injury, disability, death, or property damage arising from participation in the program.
Medical Treatment Authorization
I authorize Level Up Acro to provide first aid and, if necessary, seek medical treatment on my behalf in case of emergency. I understand that I am responsible for all medical expenses incurred.
Code of Conduct and Safety Agreement
I agree to follow all safety instructions and respect the rules set by the program staff. Failure to adhere to these rules may result in removal from the program without a refund.
Tuition
Tuition is due on the first of the month
There is no proration for missed classes, although you are welcome do do makeups while still enrolled and attending Level Up classes.
Thirty days (30 days) written notice is required to drop any class. Proration for half months will not be provided in the event of cancelation.
Registration fee
There is an annual registration fee, which is charged once yearly regardless of the number of classes taken. Once you are enrolled in a class, these are nonrefundable should you decide to cancel or leave the studio.
Level Up Acro reserves the right to cancel any class with less than 3 students
PRIVATE CANCELLATION POLICY
48+ hours' notice: no charge
24-47 hours' notice: 50% of private
less than 24 hours or no-show: full price
All payment for privates must be received the day of or a $10 fee will be added to the fee charged
