Tuition & Fees Waiver Acknowledgment

FULL SEASON PROGRAM
Tuition is billed in 10 equal monthly payments.
Payment is due by the 10th of each month.
A $15 late/billing fee will be assessed on payments received after the due date.
Season Commitment Acknowledgment
* Yes, I understand that full-season tuition covers 10 monthly payments and includes:
37 scheduled class lessons, Winter On-Stage Performance, Spring Dress Rehearsal, Spring Performance (Recital)
* Yes, I understand that if my dancer enrolls mid-season, tuition begins at the time of registration and continues through the Spring Performance.
* Yes, I understand that tuition and costume fees remain due in full unless I formally notify the studio of my intention to discontinue classes in accordance with studio policy.
Additional Required Fees:
~Costume Fees: Charged with November tuition, Non-refundable
~Family Recital Fee: $85 per family charged with January tuition

SUMMER CLASSES & CAMPS
*Yes, I understand that summer class and camp tuition is due in full at the time of registration, and space is not guaranteed until payment is received.
*Yes, I understand that tuition will not be refunded if the class is full due to other students not being able to register for the class.
