Liability Waiver and Release
I, the undersigned participant (or parent/guardian if participant is under 18), understand and acknowledge that participation in dance classes, rehearsals, performances, and any related activities at TWDC involves inherent risks of injury, including but not limited to, sprains, strains, fractures, and other physical injuries. I voluntarily assume all risks associated with participation in these activities.
I further acknowledge the risks associated with exposure to infectious diseases, including but not limited to COVID-19, influenza, and other communicable illnesses. I understand that while TWDC follows safety protocols, it cannot guarantee that participants will not be exposed to such diseases. I voluntarily assume all such risks related to infectious diseases.
I agree to observe all safety guidelines and to report any unsafe conduct or hazardous conditions to TWDC staff immediately. I understand that failure to adhere to safety policies may result in removal from participation.
I hereby release, waive, and discharge TWDC, its owners, instructors, employees, and agents from any and all claims, liabilities, demands, or causes of action arising from or related to my (or my child’s) participation in dance activities, whether caused by negligence or otherwise. I further agree to indemnify and hold harmless TWDC from any claims brought by third parties arising from my (or my child’s) participation.
I acknowledge that I (or my child) am physically fit and have no medical condition that would prevent safe participation in dance activities. I agree to inform the studio of any medical conditions, allergies, or injuries that may affect participation.
Medical Authorization
In the event of an emergency, I authorize [Dance Studio Name] and its staff to seek medical attention for me (or my child) and to transport me (or my child) to the nearest medical facility if necessary. I understand that I am responsible for any medical expenses incurred.
Media Release
I grant TWDC the right to take photographs, video recordings, and other media of me (or my child) during dance classes, rehearsals, performances, and other related activities. I consent to the use of such media for promotional, advertising, and marketing purposes, including but not limited to, social media, websites, printed materials, and other public forums without compensation.
I understand that I may revoke this consent in writing at any time, but such revocation will not apply to materials already in use.
Acknowledgment and Agreement
I have read and understand this Liability Waiver and Media Release. I voluntarily agree to its terms and conditions.
