ACKNOWLEDGEMENT OF RISK AND RELEASE OF LIABILITY
I, the undersigned, hereby acknowledge and agree that participation in dance classes and related activities at RhythmX Dance Lab involves physical exertion and carries inherent risks, including but not limited to: slips, falls, muscle strains, sprains, broken bones, and other potential injuries. I voluntarily assume all such risks and accept full responsibility for any injury or accident that may occur.
I certify that I (or my minor child/ward) am physically fit to participate in the programs and have not been advised otherwise by a qualified medical professional.
In consideration of being allowed to participate in the classes, rehearsals, performances, and other activities provided by RhythmX Dance Lab, I hereby waive, release, and discharge RhythmX Dance Lab, its owners, instructors, employees, agents, and representatives from any and all claims for injury, illness, damage, loss, or liability of any kind arising from participation, whether caused by negligence or otherwise.
GENERAL PROVISIONS
This waiver is valid for the duration of participation in RhythmX Dance Lab programs, unless revoked in writing.
I agree to follow all studio rules and instructions provided by instructors and staff.
I understand that failure to do so may result in removal from class or termination of participation.
PARENT/GUARDIAN CONSENT (If Participant is Under 18)
I, the undersigned, am the parent or legal guardian of the above-named minor. I have read and understand this waiver and release and agree to its terms on behalf of my child/ward. I also give permission for my child to receive emergency medical treatment in the event of an accident or illness.
