I freely and knowingly assume all risks incidental to participating in BelArts Ballet (a trade name of Emerald Ballet Theatre - EBT) programs during the 2025-2026 season.

I hereby waive any right, claim or course of action against BelArts Ballet, its officers, directors, employees or agents, releasing them from any liability arising out of an injury, direct or indirect.

I give permission for my child to be photographed and/or video recorded during activities with BelArts Ballet. I understand that group photos and videos may be used in BelArts Ballet’s marketing materials and social media. I also allow BelArts Ballet to use individual photos or videos of my child for publicity and fundraising purposes, unless I notify them at info@belartsballet.org requesting that individual images not be used.

I consent to receive mass communications from BelArts Ballet, including emails, text messages, and other electronic notifications regarding classes, performances, events, schedule updates, and important studio information.

I have read through and agree to BelArts Ballet's school policies posted on https://belartsballet.org/registration/.
