I, the undersigned parent/guardian of the student named below, authorize my child to leave Center Street Dance Academy (CSDA) unaccompanied after their scheduled class(es).

I understand and agree that once my child has left CSDA premises, the Academy and its staff are no longer responsible for their supervision, safety, or well-being.

I release and hold harmless Center Street Dance Academy, The Arts at Center Street, its instructors, staff, and affiliates from any and all liability, claims, or causes of action arising from my child’s independent dismissal and travel after leaving the studio.
