If you are unsure which class is right for your dancer, please contact us before registering. We are happy to help you find the best placement.

I agree to allow the use of photographs and videos of myself/ my child(ren) to be used for publicity purposes by Mendocino Ballet, and agree that such items are the sole of the Mendocino Ballet.

I agree to pay all the fees due to Mendocino Ballet. I understand fees are due by the first class of each session and or registration. I understand and agree that any make-up classes are my responsibility and must be made within 30 days of the class missed, as determined by the instructor.
I agree to the cancellation policy, that if for any reason I wish to cancel classes, I will be charged the price per class at $25/class and refunded the remainder, minus an other fees due.
All credit card payments made online and in person will have a 3.2% credit card transaction fee. This is charged by the credit card, and does not make us any money at all, but allows you the convince of paying by a credit/debit card. Feel free to pay by cash or check in person if you wish to avoid this fee.
I recognize and understand the risks of physical injury inherent to dance and dance training and am willing to assume those risks for myself/child(ren). I agree that I will not hold Mendocino Ballet (including any faculty, assistant, board member or employee) liable for injuries sustained or illness contracted by me/my family while in attendance at Mendocino Ballet. I understand that it is my responsibility to safeguard my personal property and will not hold Mendocino Ballet responsible for loss or damage of personal property while in attendance at Mendocino Ballet. I also understand that the teaching of dance requires a certain amount of physical contact between teacher and student and that such physical contact is inherent in the teaching process and not meant to be inappropriate.
Email addresses listed above will only be used by the Mendocino Ballet for billing information, newsletters, and notification of upcoming performances/events. They will NOT be given to any outside individuals, businesses, or organizations.
