The undersigned acknowledges that participation in dance and fitness programs involves an inherent risk of physical injury and assumes all such risks. Elixir Dance and Fitness LLC and its dance/fitness program, assumes no responsibility under any circumstances or at any time for the personal property
of the undersigned. The undersigned agrees that for the sole consideration of Elixir Dance and Fitness LLC, allowing the undersigned to participate and making available to the undersigned for his/her use while participating in such programs, activities, or personnel, the undersigned participant does hereby waive liability, release and forever discharge Elixir Dance and Fitness LLC, rights and causes of action of whatever kind or nature, arising out of all known and unknown, foreseen and unforeseen bodily and personal injuries, damage to or loss of property, and the consequences thereof, including death, resulting from any participation in or in any way connected with such dance/fitness programs or outreach activities located on the premises or offsite. The undersigned acknowledges that it is my responsibility to seek the advice of a medical professional if I have questions about (I ) or my (my child’s) physical ability to participate in any dance activity.
I further covenant and agree that for the sole consideration stated above I will not sue Elixir Dance and Fitness LLC, for any claim for damages in personal property or physical body arising or growing out of my (my child’s) participation in the dance/fitness programs or outreach activities. This Release, Waiver of Liability, and Covenant Not to Sue shall remain in effect for as long as I (my child) am (is) a participant in programming at Elixir Dance and Fitness LLC.

Photo and video release
I, hereby grant and authorize Elixir Dance and Fitness LLC the right to take, edit, alter, copy, exhibit, publish, distribute and make use of any and all pictures or videos taken of me or my child by Elixir Dance and Fitness LLC to be used in and/or for legally promotional materials including, but not limited to, newsletters, flyers, posters, brochures, advertisements, fundraising letters, annual reports, press kits and submissions to journalists, websites, social networking sites and other print and digital communications, without payment or any other consideration. This authorization extends to all languages, media, formats and markets now known or hereafter devised. This authorization shall continue indefinitely, unless I otherwise revoke said authorization in writing.
I understand and agree that these materials shall become the property of Elixir Dance and Fitness LLC and will not be returned.
I hereby hold harmless and release Elixir Dance and Fitness LLC from all liability, petitions, and causes of action which I, my heirs, representative, executors, administrators, or any other persons may make while acting on my behalf or on behalf of my estate.
